General emergency alarm signal must: be audible in spaces identified in paragraph (a) of this section with all normally closed doors and accesses closed. Vessel general alarm does not sound in interior spaces. Communications to be maintained via on board equipment and hand held VHF radios. Vessel is authorized a one-time transit to Port Fourchon, Louisiana, in order to complete permanent repairs to general alarm system within 7 days. 46 CFR 113.25-9(b)

Each generator must meet the applicable requirements for construction and testing in section 4-8-3 of the ABS Marine Vessel rules. Vessel experienced electrical surge event on Number 3 ship service diesel generator resulting in catastrophic failure of engine. Vessel meets minimum requirements of Subchapter J with two other operable ship service generators along with a functioning emergency generator. Vessel shall perform tag-out procedures on the number 3 generator and complete repairs within 60 days. 46 CFR 111.12-5

The lifebuoy intended to be used for quick release must have a mass of not less than 4 kg. The ring buoy's installed on the wing of each side of the bridge were actually "deck buoy's" (mass of 2.5 kg) not meeting the requirments for weight. Replace those with the "bridge buoy" type of 4 kg mass.

STBD cutlass bearing requires replacement.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules, except as otherwise provided by this subchapter. While conducting sea trials, the electo-pneumatic hydraulic system for controlling the shaft break on port shaft failed, resulting in the shaft break locking in place. Following replacement of the solenoid, Coast Guard witnessed the shaft break locking in place again. Vessel is to troubleshoot issue and make repairs as needed. After successful restoration of the control system, vessel is to contact MSU Port Arthur to clear the deficiency. 46 CFR 58.05-1(a)

Each inflated rescue boat must be repaired and maintained in accordance with the manufacturer's instructions. All repairs to inflated chambers must be made at a servicing facility approved by the Commandant, except for emergency repairs carried out on board the vessel. Vessel crew unable to prove that the recent permanent repair made to the inflatable rescue boat was performed by a company approved by the CG to do so. `

Before the vessel leaves port and at all time during the voyage, each lifesaving appliance must be in working order and ready for immediate use. One (01) liferaft was inproperly rigged for release. Owner corrected on the spot.

The navigating bridge lifebuoys are attached to the vessel's superstructure and pose a significant safety risk if deployed manually. Bridge deck lifebuoys must be stowed where they can quickly be released from the bridge and fall directly into the water without striking any part of the vessel.

Engine room door and engine room emergency escape hatch gaskets are worn and rotted. Replace gaskets and adjust each to make watertight.

The engine room bilge suction pipe exceeds a height to properly pump and drain the compartment and is not fitted with a suitable strainer. Extend the engine room bilge suction pipe and install bilge strainer.

The rescue boat fails to start or continue to run after starting. Prove proper operation of the rescue boat.

The retro-reflective tape on the rescue boat is faded and torn. Provide and attach CG/SOLAS approved reflective tape on the rescue boat.

The rocket parachute flares expire 30 Jun 2019. Replace red parachute flares.

There are currently no Lockout/Tagout procedures onboard. Provide and implement a Lockout/Tagout log and procedures in accordance with 29 CFR 1910.335

Periodic Safety Test Procedures (PSTP's) were not conducted and recorded in accordance with the CG approved PSTP's. Conduct and provide proof of completion.

Fire and man overboard drills were conducted and indicated that the crew was unfamiliar with the equipment and procedures. Conduct fire and man overboard drills.

Port Shaft seal overheated and made noise while underway transiting from New york to Delaware Bay.

1. Properly label appropriate load of each circuit on vessels panelboards.

2. Provide manufacturer plans, data reports for #1, 2, and 3 pressure vessels.

3. Remove combustible material/cabinet in stairway.

Provide approved Periodic Safety Test Procedures.

Provide corrected Fire Control Plan.
