The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces, and other hazards. One of the control source breaker from the emergency generator was malfunction. 74 SOLAS (2009) II-1/26.1

Issued 5 November 2020 Resolved
Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The break release cable from inside the starboard side lifeboat malfunctioned which pulled the cable out of the lifeboat and bent a directional bracket on the davit.

Resolution: Received class report (Record No. 20LA0086) attesting to replacment of wire on lifeboat release gear.
07 - Fire Safety › N/A - No Subsystem › Fire prevention structural integrity
Issued 5 November 2020 Resolved
Structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. MI found hydraulic line in steering space to have leaked fluid into the fire protection insulation. Insulation appears soaked with hydraulic fluid from hydraulic pipe flange down to behind electric motor controllers. In addition, aft port corner of the steering space, 6 foot section of insulation has been pulled down reducing structural fire protection in that section.
Action required: 15 - Rectify deficiencies by next port

Resolution: Received class report (Record No. 20LA0086) attesting to soiled lagging being renewed.
11 - Life Saving Appliances › N/A - No Subsystem › Immersion suits
Issued 16 July 2019 Resolved
Immersion suit shall be constructed with waterproof materials such that can be unpacked and donned without assistance within 2 minutes taking into account donning of a lifejacket if the immersion suit is to be worn in conjunction with a lifejacket. Crew was unable to dont lifejackets in conjunction with the immersion suits. 74 SOLAS (14) III/8.3 LSA CODE 2.3.1.1.1 17ac

Resolution: Vessel bought 40 new lifejackets with valid certificate. Crew tried on immersion suit with lifejackets and sent pictures. Please see attached documents.
