Company should establish procedures to ensure the ship is maintained in conformity with provisions and relevant rules and regulations. In meeeting these requirements the company should ensure that inspections are held at appropriate intervals. PSCO observed eyelets on the bracket for the gangway ladder were rusted through and failed under normal use. The monthly maintenenance checks showed it was checked one month ago and did not rust in that short of time. Recommend class Audit within next 3 months. 74SOLAS(20) IX/3.1 ISM CODE 10.2.1 705c WITHIN 90 DAYS
