Vessel is not adequately documenting the completion of their Periodic Safety tests as required by 46CFR61.40-6. 74 SOLAS (04) II-1/46.2

Vessel's records for planned/preventative maintenance and critical spare parts inventory in NSE were not up to date. SOLAS (04) II-1/46.1

Main and emergency switchboards shall be so arranged as to give easy access as may be needed to apparatus and equipment, without danger to personnel. MI identified excessive clutter in the emergency generator room that was blocking access to the back of the emergency switchboard and the phone booth. SOLAS (04) II-1/45.2

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. Life-saving appliances shall be fitted with retro-reflective material. Retro-reflective material on over half of the lifebuoys was not in a serviceable condition. 74 SOLAS (20 Cons) III/34 LSA Code 1.2.7

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. Electrical box in NO 1 lifeboat engine compartment was hanging loose and not secured.74 SOLAS (04 Cons) II-1/40

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The port lifeboat was found with a crack between a previous repair and the deck, along the aft port side IWO the seat, which had water intrustion in the crack, between the deck and the previous repair. Port Life boat is only to be used for an actual abandon ship, until the OEM or ASP has inspected the boat and determined it is safe and fully operational.

Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its life-saving appliances or other equipment, the master shall report at the earliest opportunity to the Administration or recognized organization responsible for issuing the relevant certificate, who shall cause investigations to be initiated to determine whether an additional survey is necessary. If the ship is in a port of another Contracting Government, the master or owner shall also report immediately to the appropriate authorities of the port State and the nominated surveyor or recognized organization shall ascertain that such a report has been made. Vessel master reported a loss of propulsion due to a power surge when the bow thruster was energized that caused several breakers to trip, to include the main engine lube oil pump. Upon loss of pump pressure, the main engine gave the automatic shutdown command. Vessel shall troubleshoot cause of power surge/failures with appropriate technicians. Vessel master or owner shall notify LR and local port authorities prior to departure or entry into ports.

Ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration in accordance with the provisions of regulation XI/1, or with applicable national standards of the Administration which provide an equivalent level of safety. A crack was found propagating in port side tunnel fwd of cargo hold#2, from the overhead into the bulkhead . Similar issue found on starboard side, a few frames forward from #2 cargo hold.

The Auxiliary Storage tank in the Steering Room was found cracked. The damage was previously identified and marked.

The following requirements shall govern application of the tables: Tables 9.5 and 9.6 shall apply respectively to the bulkheads and decks separating adjacent spaces. Multiple cable penetrations were found holed, while cable running work was ongoing between D, E, & F decks in the Cable Duct locker.

In addition to complying with the specific provisions for fire integrity of bulkheads and decks of cargo ships, the minimum fire integrity of bulkheads and decks shall be as prescribed in tables 9.5 and 9.6. SFP in CO2 room appears to not meet requirements. Class to survey and determine if installation is in accordance with class rules to meet SOLAS

The life-saving appliances and other equipment of cargo ships of 500 gross tonnage and upwards as referred to in paragraph (b)(i) shall be subject to the surveys specified below. The Safety Equipment Certificate, Form E, lines 2.9 - 2.9.2 state that there are 46 immersion suits, and that none comply with the requirements for life jackets. During USCG inspection, multiple suits were found which did comply with the requirements of 2.3.1.4 of the LSA Code. Class to survey the immersion suits, verify the quantities, and reissue the Form E with the correct number of suits on board which are compliant with the requirements for lifejackets.

Drills shall, as far as practicable, be conducted as if there were an actual emergency. During the abandon ship drill, the boat was rigged with fall prevention devices (FPDs), conducting the drill as maintenance, not as an actual emergency. Practice and conduct the drill for USCG during the next port call in Charleston, SC.

Per the ship’s SMS, the following cite from SOLAS is the reference for the requirement of the drill. “Except as provided in paragraphs 3.3.4 and 3.3.5, each lifeboat shall be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months during an abandon ship drill.” Additionally, the SMS references 46CFR199.180(d)(4) which states: “each lifeboat must be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months during an abandon-ship drill. During the abandon ship drill, the boat was rigged with fall prevention devices (FPDs). The vessel did not follow its SMS, which references SOLAS and 46 CFR 199.180(d)(4). Company to conduct an internal investigation, to determine the root cause of the non-compliance, then develop and implement a corrective action plan into the SMS, to be verified during the next scheduled verification by ABS.

During inspection of the CO2 system, 3 bottles were found with hoses that were loose. Hoses were wrench tightened on the spot.

Engines installed on vessels that were constructed on or after January 1, 2000, with power output of more than 130 kW must be covered by a valid EIAPP certificate, certifying the engine meets the applicable emission standards of Annex MARPOL Annex VI. Provide EIAPP Certificates for all applicable marine diesel engines as proof of emissions compliance with the IAPP. 40 CFR 1043.30

The owner or operator of a self-propelled, nontank vessel of 400 gross tons or greater, which operates on the navigable waters of the United States must prepare and submit an oil spill response plan to the Coast Guard. Vessel operator, Marine Transport Management, has submitted VRP 48562 which is not approved.
