The port lifeboat was found with a crack between a previous repair and the deck, along the aft port side IWO the seat, which had water intrustion in the crack, between the deck and the previous repair.
Port Life boat is only to be used for an actual abandon ship, until the OEM or ASP has inspected the boat and determined it is safe and fully operational.

Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its life-saving appliances or other equipment, the master shall report at the earliest opportunity to the Administration or recognized organization responsible for issuing the relevant certificate, who shall cause investigations to be initiated to determine whether an additional survey is necessary. If the ship is in a port of another Contracting Government, the master or owner shall also report immediately to the appropriate authorities of the port State and the nominated surveyor or recognized organization shall ascertain that such a report has been made. Vessel master reported a loss of propulsion due to a power surge when the bow thruster was energized that caused several breakers to trip, to include the main engine lube oil pump. Upon loss of pump pressure, the main engine gave the automatic shutdown command. Vessel shall troubleshoot cause of power surge/failures with appropriate technicians. Vessel master or owner shall notify LR and local port authorities prior to departure or entry into ports.

During USCG inspection, multiple suits were found which did comply with the requirements of 2.3.1.4 of the LSA Code. Per the ship’s SMS, the following cite from SOLAS is the reference for the requirement of the drill. “Except as provided in paragraphs 3.3.4 and 3.3.5, each lifeboat shall be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months during an abandon ship drill.” Additionally, the SMS references 46CFR199.180(d)(4) which states: “each lifeboat must be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months during an abandon-ship drill.
During the abandon ship drill, the boat was rigged with fall prevention devices (FPDs). The vessel did not follow its SMS, which references SOLAS and 46 CFR 199.180(d)(4). Company to conduct an internal investigation, to determine the root cause of the non-compliance, then develop and implement a corrective action plan into the SMS, to be verified during the next scheduled verification by ABS.

During inspection of the CO2 system, 3 bottles were found with hoses that were loose. Hoses were wrench tightened on the spot.
