During test of lifeboat engine, PSCO observed that the chief officer was unable to put the engine in neutral. 07126- Means shall be produced to control leaks of flammable liquids. The No. 6 Fuel injector on the No. 1 generator failed causing active oil leaks, covering a large portion of the top of the generator with excessive oil accumulation at the case including inside and outside of the drip pans and deck immediately surrounding the generator. Oil soaked lagging was found at the NO. 1 Generator F/O inlet valve. An active L/O leak was found on the "lube oil cooler" of the No. 2 generator causing significant oil accumulation in the drip pan. Although the No. 3 gennerator was not running, an active F/O drip was present in the vicinity of the F/O filters with oil accumulation in the drip pan. Pooled oil was found at the M/E near the base of the No. 3 & 4 cylinders.

Resolution: Generator leaks were rectified and equipment found operational as well as leak free to the satisfaction of Surveyor. Vessel provided purchase order for new gaskets to serve No. 2 generator lube oil cooler.

15109 - The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The checklist for the generators does not include requirements for visual checks of the lube oil cooler, the lube oil cooler was leaking and undetected for an indeterminate amount of time. Due to objective evidence in the above deficiencies, the vessel is not in compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under the ISM Code. An internal audit is required within 90 days to determine whether the ship is operating in compliance with the ISM Code.
