PSCO received report of separated wire rope lifeline on second tier of Port side lashing bridge, Bay 19. PSCO attended the vessel and observed the failed wire rope lifeline; PSCO expanded the exam with clear grounds and identified 03 additional wire rope lifelines with wastage/potential failure points.
