Effective protection for the crew in the form of guard rails or lifelines shall be provided above the deck cargo. PSCO received report of separated wire rope lifeline on second tier of Port side lashing bridge, Bay 19. PSCO attended the vessel and observed the failed wire rope lifeline; PSCO expanded the exam with clear grounds and identified 03 additional wire rope lifelines with wastage/potential failure points.

The Master, owner, operator, agent or person in charge of a vessel equipped with ballast tanks that operate in the waters of the US must employ one of the ballast water management methods listed in 33 CFR 151.2025. Vessel is outfitted with an approved AMS described in 33 CFR 151.2025 (a)(3) but failed to use it while conducting BW exchange. Vessel is prohibited from discharging ballast in US until one of the management req are met. 33CFR151.2025 15c

PSCO observed hydrostatic release and painter line to life rafts were not properly connected. Ship’s crew attached painter and life raft hydrostatic release unit correctly.
