This vessel is required to undergo a periodic inspection, relative to its Certificate of Inspection expiration date, noted as the anniversary date, this timeframe is three months before or after the expiration. For the Chandra B the expiration date of the Certificate of Inspection is set for October 5, 2026, making the last date of the window January 5. To facilitate this process, it is imperative for the operator to engage with the Officer in Charge, Marine Inspection (OCMI) to schedule the inspection. Coordination with the OCMI must be undertaken at a time and location that is mutually agreeable, reflecting the importance of adhering to regulatory requirements. It is essential to notify the U.S. Coast Guard (USCG) within a reasonable timeframe to ensure that the inspection can be arranged without unnecessary delays. The repetitiveness of asking for an inspection at the last minute is highly discouraged as it can lead to significant delays and complications in various processes. Vessels that operate in salt water must be have the Internal Structural Examination (ISE) inspected in accordance with the intervals set forth in table 31.10–21(a), as stated on the vessel Certificate of Inspection the ISE was due 30 April 2023. The vessel may not operate until the vessel has a completed a satisfactory Internal Structural Exam completed by a USCG Marine Inspector.

The certificates requires by 47 CFR the telecommunications regulation are expired (Bridge to Bridge, and radio certificates expired in 2020), licensees of radio stations are required to be current. Provide proof of required certificates are valid. 47 CFR 80.401

The owner, master, or person in charge shall keep records in Official Logbook or in logs or records considered to take place of the Official Logbooks of firefighting and lifesaving equipment tests and inspections, showing the dates when performed. Found no records in the official log book, of the inspection, including a statement as to the condition of lifesaving and firefighting equipment.

Before a vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Found starboard attachment and fittings line is rotten, the line strength is less than 5 kiloNewtons (1,124 pounds-force) as required by 46 CFR 199.70

A general alarm system must be installed on tank ships that meets the requirements in subchapter J (Electrical Engineering Regulations) of this chapter. General alarm pull station in the engine room is not working. The fire and smoke detection system to be serviced and demonstrate proper operation of the general alarm and fire detection system.

Arrangement of the steering gear must have either gratings or other non-slip surfaces to ensure a safe working area. Found a hydraulic leak under the console on bridge which has leaked out on to the deck. Repair the leaking hose and clean up the deck

There are no records that provide proof of the annual testing of cargo hoses. Provide proof of annual testing of the cargo hoses.

Both MDE engines are Cummins QSL9335HDIKC electronically controlled and driven by ZF bridge throttle controls, the port engine is showing a "remote analog alarm" and the port & stbd MDE is showing a water/fuel alarm. Vessel must clear all alarms and figure the cause of alarms.

Each credentialed individual must be familiar with the relevant characteristics of the vessel on which engaged this includes but are not limited to fire fighting and lifesaving equipment. When asked the master of the vessel for various information concerning the vessel he was not able to properly answer. I.e. not familiar with the operation of the smoke detection cabinet, not able to provide proof of testing of cargo hose, not able to provide the original Certificate of Inspection. Prior to operation of the vessel for commercial use he must be able to satisfactory demonstrate that he has a understating of all equipment and the status of required inspections and tests.

As the marine inspectors were not satisfied with the answers and knowledge by the crew of the onboard equipment as shown by the previous findings issued, the inspection was stopped. The crew and vessel must go through an expanded inspection prior to operation as the inspectors may at any time lawfully inspect any tank vessel and nothing in this subpart limits the marine inspector from conducting such tests or inspections he or she deems necessary to be assured of the vessel’s seaworthiness.

To maintain a valid Certificate of Inspection, you must complete your annual inspections within the periods specified in § 31.10–17 ---- "Your vessel must undergo an annual inspection within 3 months before or after each anniversary date". During a data check it was discovered that the Chandra B has not had an annual inspection between 07/26/2020 and 01/26/2021, as the anniversary date is October 26. Until the Chandra B has completed a satisfactory annual exam the vessel must be moored up and remain moored.

At least one member of the crew of a manned tank vessel shall be on board at all times except when the vessel is gas free or is moored at a dock or terminal at which watchman service is provided. Vessel does not currently have a watchman service, surveillance, and is not gas free. Vessel must provide watchman service, surveillance, or be gas free.

Each self-propelled vessel must have at least two electric generating sources. #1 main generator panel 100 amp breaker tripped while running fire pump and would not reset. Must trouble shoot and repair to like new condition.

Tank vessel shall be provided with a minimum 1 remote shutdown. Cargo pump #1 shutdown inoperable, repair to like new condition.

A fixed carbon dioxide system shall be installed in any space containing machinery using fuel having a flashpoint of less than 110 °F. Control head on CO2 bottle is loose and hoses arranged in a manner in which they are chafing. Properly service and provide documentation.

Tankships shall be equipped with independently driven fire pumps. Fire pump has significant leak, repair and demonstrate proper function to CGMI.

Portable extinguishers and their stations must be numbered in accordance with §35.40-25 of this subchapter. Number portable fire extinguishers and their stations IAW fire control plan.

Every crewmember must participate in at least one one fire drill every month. Conduct and log monthly fire drill.

EPIRB must the requirements of 47 CFR part 80. Conduct and log EPIRB test monthly.

Provide service documentation for immersion suits IAW manufacturer recommendation. Operator may remove immersion suits from vessel in lieu of servicing.

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol. An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. The marine employer shall ensure that all individuals engaged or employed on board a vessel are fully indoctrinated in the requirements of drug and alcohol program. Provide list of crew enrolled in DAPI program, alcohol testing capabilities onboard vessel, and proof of EAP training.

Cargo pump pressure gage shall be tested at least once a year for accuracy. Provide certificate/documentation that gage has been properly serviced and calibrated.

Ring buoy lifelines must be non-kinking, not less than 8 millimeters ( 5/16 inch) in diameter, of a breaking strength which is not less than 5 kiloNewtons (1,124 pounds-force, and if synthetic, a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. Provide correct color and length line for ring buoys.

Replace mechanical plug on port side containment.

Each vessel must have the following: marine charts of the area to be transited, published by the National Ocean Service, U.S. Army Corps of Engineers, U.S. Coast Pilot [name withheld] List. Provide current Coast Pilot [name withheld], and updated charts.

Port engine aft hydraulic hose to reduction gear is leaking into tray below and has automatic bilge pump. Clean area free of oil.

Aft life ring is damaged from the sun. All lifesaving appliance shall be ready for immediate use.

The company security officer must ensure a vessel security plan is developed and implemented for each vessel. Provide vessel specific security plan for vessel with proper documentation.

Vessel's fire pump gauge is not working. Must replace/fix to new condition.

Vessel must provide Periodic Safety Test Procedures (PSTP) and prove loss of power to throttle control, does not change shaft direction or increase in speed.

Vessel has/had a Internal Structural Exam date of 10 August 2018, each tank vessel must undergo an internal structural every 2.5 years. Prior to conducting any cargo operation provide a acceptable plan for and when to have USCG attend for Internal Structural Exam

Vessel does not have onboard nor does the crew have access to the USCG approved Vessel Response Plan as required. Provide proof that VRP is onboard and make available for USCG review.

Life raft is not properly connected to the weak link. Properly attach to weak link.

Demonstrate the operation of the cargo pump recirculation line in presences of USCG Marine Inspector.

Exhaust lagging for both MDE's is not covering all of the dry exhaust. Readjust or provide additional lagging.

Hydro pneumatic tank for potable water requires a relief valve by the manufacture. Tank MAWP is 100 PSI. Currently the system starts at 84 PSI and shuts off at 100 plus PSI. Adjust system to shut off prior to 100 PSI and install a relief valve IAW the manufacture.
