Every steam pipe and every fitting connected thereto through which steam may pass shall be designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. PSCO observed steam leaks from the #1 F/O heater supply regulating valve and the M/E L/O heater supply regulating valve.

Fire-fighting system and appliances shall be kept in good working order and readily available for immediate use. PSCO observed the emergency fire pump unable to provide pressure upon inital request to test. After crew troubleshooting, re-test of emergency fire pump was found to be satisfactory.
