PSCO observed discolored water coming from the following faucets: Bridge washroom, galley and wiper stateroom.

The company shold establish procedures, plans and instructions for key shipboard operations..vessel lacked procedures mandated from SMS procedures for inerting and handling cargo migration in ballast tanks and other affected areas. Cite 74 SOLAS (14) IX/3.1 and ISM Code 7.

Resolution: 07JUN21: Received report/ proper documentation from FLAG stating 01 outstanding deficiency from MISLE activity 7154031 has been rectified.
