The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed unapproved temporary repairs to the main engine lube oil plate cooling sea water intake pipe.

Resolution: New pipe was fabricated, fitted, and tested with no leakage found. NKK surveyed and confirmed that deficiency was rectified sufficiently.

Vessel currently is using the wrong form for recording training, sediment removal logs indicated in plan are not being maintained, and the plan needs an amendment to reflect new record book.
