MI observed cracked stitch weld in the portside forward crash compartment pontoon, the most upper stitch weld on the first baffle, provide repair proposal for MI review.

A hatch exposed to the weatherdeck must be watertight. MI observed scuttle gasket leading to port/stbd anchor locker and fwd centerline compartment in need of replacement due to being flat and lo longer providing adequate seal. 46 CFR 179.360

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Breaker box in engine room had missing breaker or knockout on panel. Install/replace blank cover over missing breaker.

The use of twist-on type wire nuts is permitted as long as connections are made within an enclosure. Power cable wiring for fire pump found outside junction box and reduced in wire size to accommodate installation. Provide evidence the wire size can be reduced from current size cable to smaller size wiring used for fire pump size and type. Any open wiring needs to be installed in a junction box.

Vessel was missing IBA. O/O indicated that it's currently at licensed CG repair facility for yearly servicing. Vessels operating on oceans route must have IBA for 67% of total number of persons onboard. 46CFR180.202(b)

Fire hose with nozzle must be attached to each hydrant at all times. Hose was missing do to inoperable test conducted on 19Jan2021. Replace to satisfaction of MI. 46CFR181.320(a)

EACH GENERATOR MUST BE PROTECTED BY AN OVERCURRENT DEVICE SET VALUE NOT EXCEEDING 115% OF THE GENERATOR FULL LOAD RATING.

Provide a complete listing of both the lower deck and upper deck hydraulic winch installation including all cabling, accessory gear placed on board (such as block and tackle, gear being towed, etc). 1. Vessel’s COI is for 44 persons on an oceans voyage. However, vessel is only provided with two 12 person life rafts for a total lifesaving of 24 persons. Vessel is limited to carrying not more than 24 total persons on board while operating more than 3 nautical miles from shore until additional survival craft are obtained in accordance with 46 CFR 180.202.

2. Company is conducting drug testing one month per year, which is not considered random. Company must conduct random testing throughout the year in accordance 46 CFR 16.230.

3. Provide a copy of MIS Report Confirmation of Filing Certificate for calendar year 2016 in accordance with 46 CFR 16.500

4. Provide documentation that all supervisory personnel have received appropriate one hour of training per 46 CFR 16.401 (b).

5. Provide approved alcohol testing devices in accordance with 46 CFR 4.06. Testing devices must be approved by the National Highway Traffic Safety Administration.

6. Repair HF radio antenna connection and post appropriate emergency radio frequencies in accordance with 47 CFR 80.1089.

7. Provide an FCC station license per 47 CFR 80.401.

9. Post the vessels COI and Stability letter so that all pages are visible in accordance with 46 CFR 176.302 and 176.306.

10. Provide appropriate charts for all areas the vessel operates in accordance with 46 CFR 184.420

11. Vessel has sluice valves installed for draining water from the area forward of the collision bulkhead to the sewage/water tank compartments. As sluice valves are not permitted in watertight bulkheads, the piping and valve must be permanently removed in accordance with 46 CFR 179.320.

12. Provide a portable bilge pump capable of pumping water from any compartment overboard in accordance with 46CFR182.25-10(e).

13.Conduct quarterly fire drills in accordance with 46 CFR 185.420

14.Conduct quarterly abandon ship drills in accordance with 46 CFR 185.520.

15.Post the emergency instruction at the operating station in accordance with 46 CFR 185.510

16.Vessel must have an operable fire pump prior to carrying passengers in accordance with 46 CFR 181.300.

17.Provide two additional hand red flares approved under series 160.021 in accordance with 46 CFR 180.68.

18. Vessel must have a public address system capable of broadcasting to all accommodation spaces simultaneously in accordance with 46 CFR 184.610.

19. Provide securing devices for the forward three hatches to prevent their loss in accordance with 46 CFR 179.360.

20. Provide 5 child life jackets in accordance with 46 CFR 180.71.

21. The ring life buoy floating water light must be attached with a lanyard and clip in accordance with 46 CFR 180.70.

22. Provide chafing material to prevent further damage to the hydraulic hoses located in the overhead on the main deck in accordance with 46 CFR 176.830.

23. Repair cracked welds in the port sewage tank space on frames 3, 4 and 5, and repair crack in the water tank space on frame 3 in accordance with 46 CFR 176.830.
