Each lifebuoy must be marked in clearly legible block capital letters with the vessel's name. The life buoy is not clearly marked with the vessel's name. 46 CFR 185.604(b)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The port engine hydraulic hoses are dry rotting and worn. Replace hoses to the satisfaction of the attending marine inspector. 46 CFR 182.100

Placards containing instructions for donning and use of life jackets must be posted. Placards are not posted. Post placards. 46 CFR 185.516(a)

A vessel is inspected for compliance with the standards required by this subchapter. Aft access port to steering is water saturated and rotten around the port. Submit a repair proposal and make repairs to the satisfaction of the attending marine inspector. 46 CFR 176.800(a)

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. Supervisory personnel must receive at least 60 minutes of training. There are no records indicating crew members received education and training on the effects and consequences of drug and alcohol use. Provide a roster of crew member and the details of the education and training on the effects and consequences of drug and alcohol use they received. 46 CFR 16.401(b)

Employers are responsible for all the actions of their officials, representatives, and agents in carrying out the requirements of this part. Company consortium not administering testing of its members as a single entity. Provide Letter of Regulatory Compliance to attest consortium chemical testing policy meets the intent of the regulations set forth by the Coast Guard at 46 CFR 16 and Department of Transportation at 49 CFR Part 40 to the attending Drug and Alcohol Program Inspector (DAPI). 46 CFR 16.203(a)

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. 12-volt electrical cartridge fuse overcurrent protected panel approved by MSC under 46 CFR 177.202(b)(4) replaced not in-kind or with an MSC approved panel with circuit breaker overcurrent protection. Submit a DC one line diagram with the updated 12-volt electrical panel to MSC for approval. 46 CFR 183.380(a)

Vessel must be provided with satisfactory arrangement for draining any watertight compartment. Vessel was unable to demonstrate operation of the electric bilge pumps on starboard side in forward and aft view ports. Prove proper operation of the starboard side forward and aft viewing port bilge pumps. 46 CFR 182.500

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel replaced and altered the wiring for electrical panel board during dry dock 14 Sept 2021 without OCMI approval. Provide manufacturer’s information for new panel and a summary of work completed by electrician. 46 CFR 176.700

Each circuit breaker must meet UL 489 or other standard specified by the Commandant, and be of the manually reset type designed for: inverse time delay; instantaneous short circuit protection; and switching duty if the breaker is used as a switch. Provide proof that circuit breakers meet the following for low voltage DC circuits less than 50 volts: 1) UL 489 or other standard specified by the Commandant; 2) UL 1077 if: a. Verified by an NRTL as meeting UL 1077; b. Installed in a panel board; and c. There is at least one UL-listed fuse or UL 489 certified breaker protecting the main feeder to the panel board to provide primary branch circuit protection. 46 CFR 183.380 (m)

Each ring life buoy with a floating water light must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the water light around the body of the ring life buoy. The lanyard for the vessel's ring life buoy with a floating water light is over 6 feet long. Provide a lanyard that is at least 3 feet in length, but not more than 6 feet. 46 CFR 180.70(d)(2)

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel’s first aid kit’s ammonia inhalants and iodine applicators are expired. Provide in-date ammonia inhalants and iodine applicators for first aid kit. 46 CFR 184.710

Each child sized life jacket must be stowed in a location that is appropriately marked and separated from adult sized life jackets so the child sized life jackets are not mistaken for adult life jackets. Vessel's child size life jackets were labeled as adult life jackets. 46 CFR 180.78(a)(5)

All vessels must have operable navigation lights in accordance with the Navigation Rules. Masthead and anchor lights are extinguished. Vessel restricted from operating at night and in reduced visibility conditions. 46 CFR 183.420

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Retroreflective material is faded and missing in several sections. 46 CFR 185.604(i)

The pump listed by an independent laboratory as meeting the requirements in UL 1113. 02 bilge pumps are not UL 1113 approved, starboard centerline and port aft compartments. 46 CFR 520(e)(1)

All electrical equipment and circuits must be clearly marked and identified. High level alarm circuit wiring not labeled correctly; current wiring has black as the positive and red to ground. 46 CFR 183.220(d)

On a nonmetallic vessel, the enclosures and frames of electrical equipment must be bonded together to a common ground by a normally non-current carrying conductor. Bonding conductor on port fuel tank is not sized IAW Section 250-96 NFPA 70; conductor on starboard fuel tank is chaffed through. Renew prior to credit drydock. 46 CFR 183.372(a)

Each vessel must have permanent drafts marks at each end of the vessel; or have permanent loading marks placed on each side of the vessel forward and aft to indicate maximum allowable trim and amidships to indicate the maximum allowable draft. Vessel has no draft or loading marks on the hull. Install prior to credit drydock. 46 CFR 185.602(c)

Vessel required to have one 20" or 24" ring lifebuoy fitted with 60' lifeline and floating water light attached with a corrosion resistant clip to allow quick disconnect. Waterlight missing attached line. 46 CFR 180.709(d)(2)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Starboard outboard engine failed to start. 46 CFR 182.100

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including inspection of each portable fire extinguisher, and fixed gas fire extinguishing system to check for excessive corrosion and general condition. One required class type 40:B-C extinguisher needs to be recharged.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Vessel must record monthly inspections of extinguishers.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Replace wing nuts on battery terminals with permanent type connectors.

Cable and wire for power and lighting circuits must be listed by Underwriters Laboratories (UL), as UL Boat or UL Marine cable. Ensure all cables in battery compartment are of marine grade.

A vessel on a limited coastwise route must carry six hand red flare and six hand orange smoke distress signals. Vessel had 3 hand red flare and 3 hand orange smoke distress signals. Vessel limited to a Lakes Bays and Sounds route. Vessel rectified deficiency on the spot. 46 CFR 180.68

Each ring life buoy on a vessel must carry one floating waterlight with a lanyard of at least 3 feet in length but not more than 6 feet securing the waterlight around the body of the ring life buoy. Vessel's waterlight was attached to the ring life buoy lifeline and improperly secured to the ring life buoy. 46 CFR 180.70

Based off findings from DAPI Audit conducted 08JAN2019, bring company's chemical testing program into compliance with 46 CFR 16. Compliance due date is 30 days from date of DAPI Audit.

Port outboard 250 HP Yamaha engine suffered damage due to an alleged electrical fire reported by the operator. Owner/operator shall conduct repairs to the main engine or replace main engine. Provide a servicing report from a certified Yamaha technician and prove operation to the attending USCG Marine Inspector prior to carrying passengers.

A vessel on an oceans, coastwise, limited coastwise, or great lakes route must carry: six hand red flare distress signals and six hand orange smoke distress signals; All flares onboard vessel were expired. Outfit vessel with new flares IAW 46 CFR 180.68 (a)

As appropriate for the intended voyage, a vessel must carry adequate and up to date charts of large enough scale to make safe navigation possible; Vessel charts were out of date. Get updated charts appropriate for vessel's route. 46 CFR 182.530 (c)

After completing vessel chemical testing program compliance checklist, vessel's drug and alcohol program is not in compliance with 46 CFR 16. Complete DAPI Audit with Coast Guard Inspector. 46 CFR 16

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing actions to be taken in the event of fire, heavy weather, or man overboard conditions; Vessel did not have emergency instructions posted and crew members were not familiar with actions to be taken. Post emergency instructions and conduct emergency drills in the presence of the attending Coast Guard Marine Inspector.

A vessel of more than 26 feet in length, but not more than 65 feet must carry a minimum of one life buoy of not less than 24 inches in diameter. Vessel's ring life buoy is wasted. Replace ring life buoy. 46 CFR 180.70 (a) (2)

Provide proof of registration of EPIRB.

Permanently mount EPIRB on vessel in a location where it may float free.

Renew hydrostatic release unit for EPIRB container.

Provide certificate of documentation of vessel.

Provide 3 properly stenciled 2.5 gallon fire buckets with lanyards.

Provide proof of compliance with DAPI program requirements.

Demonstrate operating bilge alarms for all monitored spaces. Vessel shall monitor all spaces and voids for water on an hourly basis by visual means while underway.

The vessel operator failed to conduct the required drydock and annual inspections before their respective deadlines and continueed to operate. COI invalid and COI inspection required before operating.

Vessel may not operate with greater than six passengers until successful completion of a COI and Drydock/ISE.
