A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protection against hazards to life, property, and the environment. The ITV COCODRIE reported an allision with catwalk at facility while departing. ITV COCODRIE must submit a TPO report attesting to the seaworthiness and root cause for the allision.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Vessel experience vibration on the port propeller while transiting the Houston Ship Channel. As such, the vessel had to reduce its speed; thus, reducing its maneuverability capabilities which caused the tug to strike Houston Ship Channel marker #114A. Vessel reported vibration on port wheel and moored at Bostco standby pilings to pass off tow before transiting to conduct repairs. Vessel shall enact permanent repairs to the satisfaction of TPO(ABS) within Houston's OCMI zone prior to resume towing operations.
