The owner or operator of an OSV shall make the vessel available for inspection by a marine inspector each time an accident occurs, or a defect is discovered that affects the safety of the vessel. Unreported damage to vessel's starboard rudder, propeller and shaft was discovered. Vessel is prohibited from moving from current position until a qualified marine inspector ensures the safety of the vessel.

Resolution: Received request from the vessel's master to transit to Alabama Shipyard for repairs. The proposal was reviewed and approved by SECTOR Mobile.

Except in emergency, no repairs or alterations to the hull or machinery, or the the equipment that affects the safety of the OSV, may be made without notice to the cognizant OCMI in the inspection zone where the repairs or alterations are to be made. When the repairs or alterations have been made, notice must be given to this OCMT as soon as practicable. Unauthorized repairs were conducted to the vessel's jacking system without CG oversight.

Resolution: Documentation was provided to the OCMI that attested to the repairs made. In addition, the Coast Inspector conducted a visual inspection of he completed repairs and witnessed NDT testing (dye pen) at the connections below the main deck. All SAT.

Drawings of repairs or alterations must be approved, before work starts, by the cognizant OCMI. Vessel sustained damage to the vessels STBD rudder and propeller. Vessel is required to submit a repair proposal, a location for repairs, and must be approved prior to commencing work.

Resolution: After all repairs were made or verified by the Coast Guard Inspector, an in-service inspection was conducted. Vessel was found to be fit for its intended route and service. COI was reissued.
