After any survey of the ship under regulations 7, 8, 9 or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the administration. Vessel found with temporary structures on the main deck that support cable handling operations; no structural plan review was submitted or accepted by the class society that issues the cargo ship safety construction certificate. Limited to, but no including bow cable fairlead, cable support roller structures, cable tensioners (found with cracks in bracket/cargo hatch deck), and #2 & #4 carousel structural load analysis for tank top underdeck structures. Electrical cables serving HPU’s in the cargo holds run through vent closures, unspooled wind turbine cable is run through cargo hatches: hatches/vents are not able to be secured in event of a hold fire. 74SOLAS(20)I/11(b)

The administration shall take appropriate steps to ensure uniformity in the implementation and application of the provisions of this part in respect of electrical installations. Vessel has (3) 440V temporary generators providing power to hydraulic power units, portable accommodations modules, cable tensioners, and various electrical loads. Temp generators on hatch have electrical cables unsupported, chafing, and run on the hatch covers and main deck leading down into #2 &#4 cargo holds. Vessel does not have class review or approval for the breakers, ampacity, or sizing of the temporary electrical equipment. Cables were found chaffed at corner points on the hatches and leading down vent trunks. 74SOLAS(20)II-1/40.2

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger… The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. Vessel has 3 temporary generators placed on cargo hatch 3, connected to fuel tanks by flexible fuel hose that is unverified to meet fuel piping requirements and not supported appropriately. Fuel hose is on deck in contact with electrical cables. Found 02 HPU’s located in number 2 and number 4 cargo spaces (not machinery spaces). Additional 02 found on deck. All ships which undergo repairs, alterations, modifications and outfitting related thereto shall continue to comply with at least the requirements previously applicable to these ships. The vessel has multiple portable accommodation containers on the deck. The modules are not certified as class approved in accordance with ABS guide for portable accommodation modules. The modules have electrical cables running through windows. These accommodation spaces are not listed on the general arrangement plan or fire control plan. The modules do not have smoke/fire detection, or PA speakers. The ship’s PA is unable to be heard from inside the module in case of an emergency. The general arrangement plan does not reflect the temporary equipment placed on the deck, no review or approval for fire fighting equipment is on the plan or approved by class. 74SOLAS(20)II-2/1.3
