The master, owner, operator, agent, or person in charge of a Vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the authorized ballast water management methods listed in 33 CFR 151.2025(a). Vessel's ballast water management method is inoperable. Vessel has been granted an extraordinary circumstance to employ the ballast water management method listed in 33 CFR 151.2025(a)(3) to conduct cargo operations while in the Sector New Orleans COTP zone. In accordance with 33 CFR 151.2040(b)(3), the vessel must discharge only that amount of ballast water operationally necessary to ensure the safety and stability of the vessel for cargo operations. 33 CFR 151.2025 (a)

74 SOLAS (14 cons) III/7.2.2 Lifejackets shall be so placed as to be readily accessible. The 06 lifejackets stowed in a container with a seized opening hatch did not allow access to the lifejackets in the forward hold.

Marpol Annex I/ Reg 31- Oil tankers of 150GT and above shall be equipped w/ an ODME to prevent discharge of effluent into the sea; OVBD valve is physically closing but system is not recognizing the valve as being closed.

The master, owner, operator, agent or PIC of a vessel equipped with ballast tanks that operates in waters of the United States, must employ 1 of the following methods: Alternative Management System (AMS), that meets the requirements of 33CFR151.2026 of this subpart, may also be used, so long as it was installed prior to the date that the vessel is required to comply with the Ballast Water Discharge Standard (BWDS) in accordance with 33CFR151.2035(b). Vessel's installed AMS has been inoperable since 18JUL2017.
