Marpol Annex I Reg 31.2; Code 1740; The oil discharge mon and contr sys shall operate when there is any discharge of effluent into the sea and shall be such as will ensure that any discharge of oily mixture is automoatically stopped when the instantaneous rate of discharge of oil exceeds that permitted by regulation 34 of the annex. PSCO observed alarm malfunction window indicating unable to oper.

Solas 2014 CH1 Reg 11(a); The condition of ship & its equip shall be maintained to conform w/ the provisions of the present reg to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons. PSCO observed a rise in floor elevation in ECR. The rise is approx 2"x1' in front of switchboard and at new enterance of ECR. The deck poses a safety risk.

Resolution: Subflooring concrete was buckling it was removed and rubber matting was placed back down and tape was placed over the seams until a new install of flooring is placed on the deck.

Solas (2014) CHII-2 Reg 4.5.5.3.2; The inert gas system referred to in paragraph 5.5.3.1 shall be designed, constructed, and tested IAW the fire safety systems code. PSCO observed unclear instructions and crew unfamiliar with procedures for calibration of oxygen analyser.
