No person may operate a vessel unless that person complies with all limitations in the endorsement on the vessel's certificate of compliance (COC). Vessel's COC states that vessel is authorized to carry and transfer cargo in U.S. waters in accordance with it's subchapter O endorsement(SOE). Subject's vessels aft mast riser does not comply with 46 CFR 154.805(e) and IBC 8.2.10. Until the vessel makes permanent modifications at next drydock, SOE section 12.a requires that "the crew must be familiar with the Exmar Ship Management interim safety procedures regarding the control of cargo tank pressure. PSCOs boarded subject vessel while vessel was on cargo waiver and making preparations to commence loading operations. There was no objective evidence that "permit to work - emergency preparedness/mitigating actions in case of gas release from aft mast", revision 8, had been completed for this cargo operation. 46 CFR 154.1808 10c

Electrical installations should be such as to minimize risk of fire and and explosion from flammable products. Airlock alarm on cargo deck area outside airlock door found with cracked housing, compromising the integrity of the enclosure. PSCOs observed visible water inside the fixture.
