Gas detection equipment should be so designed that it may readily be tested. Testing and calibration should be carried out at regular intervals. Suitable equipment and span gas for this purpose should be carried on board.
Ship's inventory stated that two span gas bottles should be on board. Crew provided one bottle that was nearly empty with no other suitible span gas on board.

Electrical Installations should be such as to minimize the risk of fire and explosion from flammable products.
PSCOs found two explosion proof lights to no longer be safe for operation in the gas dangerous zone posing a threat of fire and explosion. The light outside the compressor room door was found to have standing water inside and the light on the starboard forward side was found to have electrical tape wrapped around the outside and also had standing water inside the light.

Resolution: Received service report, endorsed by RO (LR), stating that the MARV had been tested and re-sealed.
Gas detection equipment acceptable to the Administration should be provided (IGC 13.6.1). Alarms should be activated when the vapour concentration reaches 30% of the lower flammable limit (IGC 13.6.10).
Fixed gas detection system for hold spaces has sensor with readout of % LEL inside cabinet and then LEL readout display on exterior of cabinet. When gas was applied, alarm activated but LEL readout on the exterior of the cabinet remained at 0% LEL. LEL readings are not being transmitted from the LEL detector to the display on the exterior of the cabinet.

Resolution: Crew fixed electrical issue and exterior panel now reads close to interior reading. Exterior panel reads 1.3% LEL when interior detector reads 0.0% LEL. Crew will continue to work on to make readings the same.
#01 –1160- Holds and Tanks -Cargo tank pressure relief valve vent exits should be arranged at a distance at least equal to B or 25m, whichever is less, from the nearest air intake or opening to accommodation spaces, other gas safe spaces. The vent mast for cargo tank #4, aft of the electric motor room does not meet 25m distance requirement. Flag equivalence determined insufficient evidence of compliance with 46CFR154.32.This deficiency is issued with a due date of 6 months form the vessel’s arrival to the US in accordance with MSC Letter C1-1703350 dated 22SEP17. #02-1138- Liquefied Gases in Bulk – A gas carrier shall comply with the requirements of the International Gas Code. Electrical equipment should not be installed in hazardous location unless it conforms with the standards not inferior to those acceptable to the Organization (IEC60092-502). PSCOs observed one explosion-proof light located in gas dangerous zone above #2 tank dome to have significant amount of salt inside light. #03 -2535- ISM- The company should ensure that all personnel involved in the company’s SMS have an adequate understanding of relevant rules regulations, codes and guidelines. Company (Exmar Shipmanagement, NV) was issued MSC Letter CI-1703350 on 22SEP17 regarding after mast riser location not complying w/ 46CFR154.805. Paragraph 3 of this letter requires that “if any of the vessels will be loading cargo in the US prior meeting (the requirements of 46CFR154.805) then the vessel must have onboard procedures to respond to tank venting, and the crew must demonstrate familiarity with these procedures to the satisfaction of the [name withheld] asked by the attending PSCOs, vessel crew was unable to present an onboard procedure. After some time a procedure was presented for the sister vessel LEVANT, but this procedure had not been read or signed by the vessel’s crew. Develop a ship-specific onboard procedure to respond to uncontrolled tank venting and ensure all crew members are familiar with this procedure. 74SOLAS(14CONS)IX/3.1 IAW ref: ISM Code 6.4 17c
