Fire protection systems and fire-fighting systems and appliances shall be maintained ready for use. PSCO observed 04 water mist nozzles with no fluid within the glass activation ampules. One sprinkler head was test broken to ensure there was water flow and the nozzle was still capable of dispersing water as designed. Water did successful disperse. However, it is unknown if the nozzle would properly expand and break in the event of a true fire, making it undetermined on its readily available service. All nozzles that were found in this condition were immediately replaced with new nozzles, and the cause is suspected to be micro-fractures win the bulb itself. 74 SOLAS (09) II-2/14.1.1. A minimum of two two-way radiotelephone apparatus for each fire party for fire-fighter's communication shall be carried on board. Those two-way portable radiotelephones shall be of an explosion-proof type or intrinsically safe. PSCO observed fire teams equipped with Motorola DP4400e radios. Label states that radio is intrinsically safe when used with Motorolla PMNN4489A. the actual battery in use was found to be Cutrate Batteries P/N M4121H1-A. At the time of exam no objective evidence was available onboard showing the athis battery model maintains the intrinsically safe capability of the radios. 74SOLAS20 II-2/10.10.4 - 50ac

Passenger ships carrying more than 36 passengers shall be equipped with automatic sprinkler of an approved type complying with the requirements of the Fire Safety Systmes Code in all control stations, accommodationa nd service spaces, inclusing corridors and stairways. PSCO observed items in rope storage locker number 5002 stacked above the height of the high-pressure water mist nozzles. 74SOLAS20 II-2/10.6.1.1

Structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions, shall be kept in good order so as to ensure their required performance if a fire occurs. Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, double-leaf doors equipped with a latch necessary for their fire integrity shall have a latch that is automatically activated by the operation of the doors when released by the system. FSD 04.5.36 was unable to close due to first double leaf door not latching, preventing the release of the second door. Door was adjusted before departure and closed without issue. 74 SOLAS 20 II-2/9.4.1.1.5.12

General arangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the... means of access to different compartments. The fire control plan identifies fire screen door 11.6.10 as an "A-Class" self-closing door. However, the door is not fitted with a self-closing mechanism. Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. PSCO observed multiple extension cords daisy chained together to charge tool batteries and radios in the BOSUN stores on deck 6. SOLAS 20 II-1/40.1.3 10c

Door and door frames in "B" class divisions and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that of the divisions. B-15 door for the deck 12 passenger laundromat did not latch upon closure, preventing the boundary from being maintained. SOLAS 04 II-2/9.4.1.2.1 10c

Fire doors in main vertical zones bulkheads, galley boundries and starway enclosures other than power - operated watertight doors and those which are normally locked shall satisfy the follwoing requirements: (1) The door shall be self-closing and capable of closing with an inclination of up to 35 degress. 74 SOLAS (20) II-2/9.4 Fire door FSD 02.6.01 in HFO separator room did not close automatically. Crew adjusted closure and ops tested sat in the presence of the PSCO.

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release and must no be equipped with any locking device. 74 SOLAS (20) II-2/13 FSD 175.02 in the youth center on deck 17 was found with a locking device. Provide documentation of adequate egress from space.

Fire doors in main vertical zone bulkheads and stairway enclosures shall close in no less than 40s and no more than 10s. SOLAS 09 II-2/9.4.1.1.4.2 PSCO's discovered the following FSD's that closed in less than 10s: FSD's 15.4.17, 16.6.14, 16.3.14. Action taken: 16 ac

A self-contained emergency source of electrical power shall be provided for a period of 36h, emergency lighting in all service and accommodation alleyways, stairways and exits. Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures other than power-operated watertight doors and those which are normally locked shall satisfy the following requirements; the doors shall be self-closing and capable of closing with an angle of inclination of up to 3.5 degrees opposing closure. 74 SOLAS 2009 CONS. II-2/9.4.1.1.4.1 PSCO discovered Fire screen door 01.4.03 handle was damaged and did not fully latch. Action taken: 10 c
