The master or officer in charge of a navigation watch must ensure all hatches and openings of the hull and deck are kept tightly closed except: when operating on lakes, bays, and sounds, without a tow during calm weather, and only if the master determines that the safety of the vessel is not compromised. Vessel operates in all conditions with engine room watertight hatch open since the engine room is not outfitted with any ventilation. Vessel is permitted to operate with engine room hatch open while conducting operations within Mare Island but must install sufficient engine room ventilation and ensure watertight hatch closure when operating under tow outside of Mare Island. 46 CFR 140.610

Employers must ensure that they and their crewmembers meet the requirements of this part (Chemical Testing). A current list of crewmembers subject to chemical testing was not available during inspection. 46 CFR 16.203(a)(1)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Grab line for starboard side lifebuoy was found deteriorating and not in serviceable condition. 46 CFR 141.230

Stowage positions for lifejackets must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. The equipment may be identified in words or with the appropriate symbol from IMO Resolution A.760(18). Lifejacket storage compartment found with none of the required markings. 46 CFR 141.340(h)

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. All engine room portholes have a small gap in the gasket, open to the exterior of the vessel, compromising watertight integrity. 46 CFR 144.320(b)

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. Watertight hatch for engine room did not have required markings. 46 CFR 144.160(e)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. Lazarette high bilge level alarm was found constantly alarming in error on E/R alarm panel. 46 CFR 143.230(a)(5)

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for various periods of time based upon the endorsements the mariner holds. Medical certificate for the Captain was not available. 46 CFR 10.301(b)(2)

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Port engine hydraulic-start was found leaking at fitting. 46 CFR 143.220(a)

Each towing vessel must be equipped with at least two portable, battery-powered lights. One must be located in the pilothouse and the other at the access to the engine room. Portable light at engine room access was found inoperable. 46 CFR 143.410(c)

Each lifebuoy with a floating electric water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the lifebuoy. Both port and starboard side lifebuoy water lights were found secured around the railing, not each lifebuoy. 46 CFR 141.360(c)(4)

The Coast Guard inspector or surveyor conducting this examination must: Examine the exterior of the hull, including bottom, sides, headlog, and stern, and examine all appendages for damage, fractures, wastage, pitting, or improper repairs. MI observed the following: 1. Cracked welds located along the chine approximately 3 feet aft of port side bow, and below port side keel cooler. 2. Severe deterioration and wastage to struts and transverse support beam below port and starboard tail shafts. Wastage extends from aft edge forward of beam approximately 3". 3. Excessive pitting on port side vertical keel cooler brackets. Additionally starboard side keel cooler vertical brackets and bottom frame found pitted and wasted. 4. Wasted side shell plate above middle of starboard keel cooler. 46 CFR 137.330(a)(1)

The Coast Guard inspector or surveyor conducting this examination must: Examine the rudders for damage, the upper and lower bearings for wear, and the rudder stock for damage or wear. MI observed the following: 1. Severe pitting and wastage to both port and starboard rudders along leading edge extending aft of rudder stock approximately 2". 2. Cracked vertical weld on aft edge of port side rudder. 3. Wasted metal on bottom of both port and starboard rudders below box fitted to aft edges. 4. Cracked horizontal weld outboard of starboard rudder post along rudder shoe. 5. Heavily pitted and corrded metal on rudder post flanges affixed to the rudder shoes. 6. Severe wastage and pitting to shaft access hole penetration pipe on port and starboard rudders. 46 CFR 137.330(a)(3)

The Coast Guard inspector or surveyor conducting this examination must: Examine the propellers for cracks and damage. Marine inspector observed deterioration to keyways for both propellers and damaged threads on shaft. Remove both port and starboard propellers for further inspection. 46 CFR 137.330(a)(4)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Marine inspector observed wiring for shore power in storage space below operating station terminating in dead end wire. 46 CFR 143.400(g)

An internal structural examination required by this part may be conducted while the vessel is afloat or while it is out of the water. Marine inspector observed deteriorated weld in lazarette on inboard side of overhead pipe at aft bulkhead 46 CFR 143.400(b)

Towing vessels must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72, as applicable. Vessel's VHF radio was in operable and missing GPS and MMSI number. 46 CFR 140.715

Each lifebuoy on a towing vessel must be approved under approval series 160.050 or 160.150. SOLAS retro-reflective tape was deteriorated and missing from vessel's port and starboard ring buoys. 46 CFR 141.360(b)(1)

Each fire alarm are approved and the system is installed, tested and maintained in accordance with the manufacturer's design manual. Engine room fire alarm was insufficiently mounted.

The hull of each documented vessel must be marked as required by part 67 of this chapter. Vessel's name was insufficiently painted on the bow. 46 CFR 144.160(a)

The strength of each component used for securing the towing vessel to the tow and for making up the tow is adequate for its intended srevice. MI observed an impact crunch in the forward main deck wire. 46 CFR 140.801(a)

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. There are no records of any navigational assessments being completed on the vessel.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. Vessel could not provide evidence of crew being in compliance for drug testing.

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crewmembers are capable of performing the duties expected of them during emergencies. This includes abandoning the vessel, recovering persons from the water, responding to onboard fires and flooding, or responding to other threats to life, property, or the environment. There are no records of drills being conducted on the vessel.

Each vessel must be designed and constructed to minimize fire hazards insofar as reasonable and practicable. Numerous cable penetrations onboard are not vapor tight. Make cable penetrations vapor tight to the satisfaction of the OCMI. 46 CFR 144.405

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector or TPO, as applicable. Fire extinguisher service report is unavailable. Provide servicing report to the satisfaction of the OCMI. 46 CFR 142.240(a)(1)(IV)

A fixed fire pump must be capable of delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot-tube pressure of at least 50 pounds per square inch (psi). Fire pump is missing a gauge. Install pressure gauge on fire pump. 46 CFR 142.325(a)(1)

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Fuel Shut-off valve is not properly labeled. 46 CFR 143.260(e)

The Coast Guard inspector or surveyor conducting this examination must examine the rudders for damage, the upper and lower bearings for wear, and the rudder stock for damage or wear. Both port and stbd rudder post cutlass bearings are severely worn and leaking water while vessel is underway. Replace cutlass bearing to the satisfaction of the OCMI. 46 CFR 137.330(a)(3)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Employee list is not onboard. Provide list of employed crewmembers who are enrolled in the company's Drug and Alcohol program. 46 CFR 16.210(a)

Each lifebuoy on a towing vessel must be marked in block capital letters with the name of the vessel. The port side lifebuoy is not labeled with the vessel's name. Label lifebuoy. 46 CFR 141.360(b)(4)

Each storage container containing a work vest must be marked “WORK VEST”. Storage container is not properly labeled. Label storage container for work vests. 46 CFR 140.430(b)

No later than July 22, 2019, the owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include recordkeeping procedures. Health and safety plan is missing. Provide evidence of the health and safety plan to the satisfaction of the OCMI. 46 CFR 140.500(a)

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. Vessel's towing record is missing. Provide evidence of towing record to the satisfaction of the OCMI 46 CFR 140.820(b)
