All electrical apparatus shall be so constructed and so installed as to not cause injury when touched in a normal manner. PSCO observed 03 different power strips daisy chained together as well as 02 different power wires spliced together and secured with electrical tape powering the INMARSAT equipment and NavBox within the Electrical Room on the bridge. 74 SOLAS (14) II-1/45.1.3

Arrangements shall be provided to enable the pilot to embark and disembark safely on either side of the ship. PSCO's received a report from the Maryland Pilots that the vessel's pilot ladders were too short and were not properly secured to the vessel deeming the ladders unsafe for their use. PSCO's observed photos taken by the pilot and confirmed the pilot ladders did not meet IMO standards. SOLAS (2020) V/23.3.3.1
