Every ship of 400 GT and above shall carry a garbage managemnt plan in accordance with the guidelines developed by the Organization. Last approved garbage management plan is from 2009 and does not address new IMO guidelines. MARPOL V/9 IMO IA656E

Liferaft painter system shall be arranged to ensure that the liferaft when released is not dragged under by the sinking ship. Liferaft painter was improperly arranged. 74 SOLAS (97) III/38.6.1

Suitable instructions and component spares for testing shall be provided to test fixed fire detection system. No spare smoke testing cannisters on board at time of inspection. 74 SOLAS (97) II-2/13.1.12

Except as specified in paragraph 3 of this regulation, any ship of 400 gross tonnage and above but less than 10,000 gross tonnage shall be fitted with oil filtering equipment complying with paragraph 6 of this reg. Each ballast water management system (BWMS) manufactured under Coast Guard approval must have a nameplate which is securely fastened to the BWMS and plainly marked by the manufacturer with the information listed in paragraph (b) of this section. The BWMS does not have the approval nameplate. 46CFR162.060-22

After any survey of the ship under regulations 8, 9, or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. The MSD is not operating as originally certified. The crew has created an unauthorized modification to the MSD by arranging for the grey water produced by the unapproved installation of a workshop air conditioning unit to drain into an open-top 55-gallon barrel via a flexible hose, which is connected by way of another flexible hose to the MSD’s vent hard pipe. The bilge well has thick oily residue from prior use as storage for oily waste, which creates a fire hazard and is an unsafe practice not authorized by the IOPP. The steering gear room was found without nonslip surfaces required in the event of an oil spill.

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids and means shall be provided to limit the accumulation of flammable vapours. The bilge was coated in a thick oily residue on the structural members and frames forming the bilge wells creating a significant fire hazard.

*DEFICIENCY WRITTEN TO 33 CFR* No person may operate any vessel with installed toilet facilities unless it is equipped with an operable Type I or Type II device that has a label on it under 159.16 or that is certified under 159.12 or 159.12(a). A strong pungent odor and statement by the chief engineer the vent pipe is clogged indicates there is an issue with the MSD not operating as initially certified.

*DEFICIENCY WRITTEN TO 33 CFR* When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the EPA under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. The MSD skin valve was not secured in any way to prevent discharge of sewage. The chief engineer stated the MSD discharges automatically when the volume of sewage level reaches a set point regardless of where the vessel is, in port or not, and the overboard has not been properly secured in years.

The steering gear compartments shall be: provided with suitable arrangements to ensure working access to steering gear machinery and controls. These arrangements shall include gratings or other nonslip surfaces to ensure suitable working conditions in the event of hydraulic fluid leakage. The steering gear compartment deck is bare steel and is not covered with gratings or nonslip surfaces.

In all ships general arrangement plans shall be permanently exhibited for the guidance of the ship’s officers, showing clearly for each deck the fire detection and fire alarm systems. The fire control plan indicates the vessel was built to method IC which requires smoke detection and manually operated call points in all corridors, stairways and escape routes within accommodation spaces (74 SOLAS 97 II-2/52.1). The fire control plan does not indicate where any smoke detectors should be installed.

All ships of 150 gross tonnage and upwards in addition to the requirements of paragraph 2.1, be fitted with: a daylight signally lamp, or other means, to communicate by light during day and night using an energy source of electrical power not solely dependent upon the ship’s power supply. The crew stated the daylight signalling lamp does not have a secondary source of power in accordance with IMO Resolution MSC.95(72)7.3.

A Vessel General Permit (VGP) provides National Pollutant Discharge Elimination System (NPDES) permit coverage for discharges incidental to the normal operation of all non-recreational vessels of 79 feet or greater in length while operating in U.S. waters. This permit is effective from December 19, 2013 to December 18, 2018 (i.e., five years). The vessel has a valid NPDES permit, but the crew could not locate the VGP and were unfamiliar with its contents, such as incidental discharges and visual inspections.

0690 - Before a ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. PSCO observed that all 4 rockets for the line throwing appliance expired in November of 2016. - SOLAS 74 (cons) III/20

A second source of energy shall be provided for an additional three starts whithin 30 minutes unless namual starting can be demonstrated to be effective. PSCO found that the emergency generator's second means of starting, using a match and film, was inoperable. 74SOLAS(04) II-1/44.2. 17ac

Posters or signs shall be provided on, or in the vicinity of survival craft and their launching controls. PSCO found that the lifeboats operating and launching instructions were missing from the embarkation station. 74SOLAS(14) III/9.2. 16ac

Simple Operating instructions with block diagram showing the change over procedures shall be permanently displayed on the navigation bridge. PSCO found that thre were no instructions for operation of remote steering gear control systems and power units or block diagrams.

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order. PSCO found the starboard liferaft cradle to be wasted.

The Ballast water management plan must include detailed fouling maintenance and sediment removal procedures. PSCO found no fouling maintenance procedures. 33CFR151.2050(g)(3). 50ac

For Each breating apparatus a fireproof line of sufficient length and strength shall be provided capable of being attached by means of a snaphook. PSCO found there to be no fireproof line with snaphook.

Gyro compass reported inoperative on letter of deviation request.
