The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. Valve is not located 12 inches away from operators seat. Make proper alterations to meet the regulation.

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Electrical wire from starter shows signs of damage. Replace with in-kind equipment.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Label life jacket compartments IAW the requirement.

Small batteries. Each small battery installation must be located in a well ventilated space and protected from falling objects. Install covers.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information.(1) Date of the training; and (2) General description of the training topics. Vessel did not have a general description of MOB/Fire drills. Implement these requirements in future trainings.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Implement these requirements in future trainings.

An adult life jacket must be provided for each person carried on board a vessel. 02 adult life jackets were no longer serviceable and destroyed. Replaced in kind with new life jackets from owners storage. 46 CFR 180.71 (a) Code 10 c

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Ibuprofen and eye wash were expired, replaced in kind with items in stores. 46 CFR 184.710 Code 10 c

A vessel must carry one floating water light unless it is restricted to day time operations. Vessel does not currently have a life buoy light that works and is restricted to day light operations until it can provide proof to the satisfaction of the attending MI. 46 CFR 180.70 (d)

The alcohol testing of each individual must be conducted with in 2 hours of when the SMI occurred. Vessel owner had expired alcohol testing strips. Marine employer is required to have alternate means of testing after post casualty until new strips can be provided to the satisfaction of the attending MI. 46 CFR 4.06-3 (a)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jackets were mixed in with adult. 46 CFR 180.78 (a)(5)

Stowage-Each flare carried to meet this section must be stowed in one of the following: A portable watertight container marked as required by 185.614 of this chapter, carried at the operation station. Vessel had properly stowed and marked flares but also had expired flares in the stowage container. Expired flares must be kept separate of the required ones. 46 CFR 180.70 (e)

A vessel not required to have a power driven fire pump must have three 2 1/2 gallon fire buckets with an attached lanyard satisfactory to the cognizant OCMI. Vessel had one of three fire buckets with no lanyard. 46 CFR 181.610

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Ring buoy does not have vsl name. Mark vessel name on ring buoy.

A vessel must be provided with a main steering gear that is: Of adequate strength and capable of steering the vessel at all service speeds. Bottom bushing on starboard air rudder cracked. Replace bushing.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Replace globe on all-round white light.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Marine Inspector observed cracking on forward gunwale cap at junction w/ raised deck.

Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Drills have not been documented for approximately nine months.

A life preserver shall be of a highly visible color, such as Indian Orange, International Orange, or Scarlet Munsell Red. 11 adult and 2 child lifejackets were faded and deteriorated beyond usable service. Passenger capacity reduced by 11 until renewed.

Ring life buoys shall be of first class workmanship and free from any defects materially affecting their appearance or serviceability. Ring life buoy faded and cracked to such an extent rendering it no longer serviceable.
