OIL FILTERING EQUIPMENT SHALL PROVIDE ARRANGEMENTS TO ENSURE THAT ANY DISCHARGE OF OILY MIXTURES IS AUTOMATICALLY STOPPED WHEN THE EFFLUENT EXCEEDS 15 PPM. THE THREE WAY VALVE IS STUCK IN THE OVERBOARD POSITION WHICH MAKES THE AUTOMATIC CONTROL INOPERABLE.

Resolution: PCSO attended the vessel to ensure that the OWS three-way valve was in good working order.
