Before a ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Both liferafts onboard vessel had incorrect hydrostatic release units installations where painters attached to liferaft cradle instead of weak link, meaning liferaft would inflate but remain attached to the ship in the event the ship sank.

Means shall be provided to control leaks of flammable liquids. Excess oil is present in ER. Approximnately 75% of bilges have a layer of oil (either fresh or weathered) under main engine, CPP tank, casecade tank, lube oil strainer, iwo remote quick closing valve #2 Stbd, and oily water in aftermost bilge well. In addition, pipes and fittings iwo these locations have oil saoked lagging.

The condition of the ship and its equipment shall be maintained. THe heat exchanger (plate type) for LT/HT Cooling Water for the Main Engine is leaking. It appears to have been leaking for a period of time as corrosion has built upon plates and bulkhead by containment drainage has been repainted where water is running to bilge.
