PSCOs found approximately 100 Annex II sample bottles all stored together without cell division, causing glass on glass contact. PSCOs also found multiple unlabeled sample bottles full of product.

For tankers of >20,000 DWT, the protection of the cargo tanks shall be achieved by a fixed inert gas system IAW the requirements of the FSS Code.
Instrumentation should be fitted for continuously indicating and permanently recording the pressure of the IGS supply and O2 content.
Vessel is a 49k DWT crude/product tanker. Instrumentation on IG panel is currently only recording O2 content; pressure is not being recorded.

Resolution: Vessel installed new ink cartridge for pressure recording device.
Electrical installations shall be such as to minimize the risk of fire and explosion from hazardous materials.
PSCOs noted 1 explosion-proof light in cargo deck area (on forward superstructure, middle) with standing water inside light. This provides evidence that lights is no longer functioning as originally designed.
