AT EACH LOCATION THERE SHALL BE CLEAR INSTURCTIONS RELATING TO THE OPERATION OF THE SYSTEM HAVING REGARD TO THE SAFETY OF PERSONNEL. INSTURCTION FOR C02 RELEASE IN ALL CONTAINER HOLD CABINETS WAS UNCLEAR AND MISSING INSTRUCTIONS TO OPERATE THE CYLINDER CONTROL CABINET FIRST. OPERATING MANUAL FOR C02 FIRE EXTINGUISHING SYSTEM IS ALSO UNCLEAR TO USE THE CABINET FIRST. IN ADDITION STEP 5 OF THE INSTRUCTIONS IS TO "SECURE MACHINERY AND FUEL SUPPLY" WHICH ARE NON-EXISTENT IN THE CARGO HOLDS.

Resolution: Actions have been taken - Both instructions (for P.O.D Cylinder Control Cabinet and Valves Control Cabinet) are combined in one and installed on each Valves Control Cabinet as per advice of USCG inspector to be more clear. Also Step 5 was removed.

SHIPBOARD SMS SAFETY FAMILIARIZATION HAS BEEN SIGNED BY CHIEF ENGINEER ON 18APR2024 STATING THEIR KNOWLEDGE OF FIREFIGHTING DUTIES AND HAS BEEN ONBOARD. 2ND ENGINEER PROVED KNOWLEDGEABLE IN RELEASING THE SYSTEM AND DEFICIENCY HAS BEEN CLEARED ON THE SPOT.

For all ships the means of embarkation shall be maintained in suitable condition for their intended purpose, taking into account any restrictions related to safe loading. PSCO attended vessel to determine extent of damage to starboard side embarkation ladder and arrangement. PSCO unable to embark vessel due to weather and high winds, however damage was examined from the pier. Starboard embarkation arrangement at the superstructure has been damaged due to surge from vessel traffic, and is completely inoperable. Starboard pilot arrangements and embarkation ladder is working satisfactorily and allows embarkation of the vessel. 74 SOLAS (2014 Cons.) II-1/3-9.3 - 16 a/c
