accumulation of combustible and flammable materials and
liquids insofar as practicable. Identified excessive amounts of oil throughout the engine room bilge, up to 12 inches of almost straight oil with minimal to no water, adjacent to the fuel trap in the centerline of the bilge, with oil also beneath the generators and main engines, posing an imminent hazard to the crew & environment, a serious substandard condition not being pro-actively managed. Remove all oil and clean the bilge in engine room, before vessel movement. 46CFR142.220(a)

All fire detection systems must be serviced every 12 months. The fire detection panel upon arrival was missing both emergency power batteries which renders the panel useless if main power is lost, and in addition, no records of annual servicing could be found since the system was installed by a P.E. in 2012. Furthermore, crew had no means to test the fire detection system, obtain the proper method per the fire detection manufacturer manual. A proper servicing company needs to service the system, before vessel movement. 46CFR142.240(a)

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. The only emergency lighting on the vessel was a dim/useless light near the fuel trap facing the bilge and the galley, and no sufficient lighting to help the crew escape from the bottom to the top of the engine room, nor lighting in the overnight bunk spaces for crew. Install at these areas and test under emergency power, prior to operating the vessel under its own power. 46 CFR 143.410

The master must ensure that all hatches, doors, and
other openings designed to be watertight or weathertight
function properly. Both the port and starboard shaft packings were leaking water at a steady rate, approximately a cup of water every 15 seconds, while the engines and shafts were stationary not under power, creating excessive water into the bilge. Tighten the shaft packing properly as designed. 46 CFR 140.610(b)
