Oil fuel pipes, which, if damaged would allow oil to escape from a storage, settling, or daily service tank having a capacity of 5001 abd above situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCO discovered the MD service tank (F0102V) valve, MDO settling tank (F0412V), and HFO sevice tank (F0109) test valve for the quick closing valves were in the closed position, not allowing the quick closing valves to be closed from a safe position outside of the space. All valves were tested on 25APR2020. HFO service tank valve (F0109) was last used before 23JUN2020 and the MDO service tank valve (F0412V) was used after 23JUN2020 and currently being used to fuel ship's boiler. Crew immediately corrected the error upon the PSCO pointing it out.

The company amd the ship shall comply with the requirements of the ISM Code. The company should establish procedures, plans, and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship, and protection of the environment. The various tasks should be defined and assigned to qualified personnel. There are no procedures for testing quick closing valves ensuring valves are returned to the correct position after testing.
