The machinery, boilers and other pressure vessels associated piping systems and fittings shall be of a design and constructoin adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum and danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. 74SOLAS(2014)II-1/26.1 Over a course of four days a steam pipe flange gasket started failing and grew from a drip to streams of steam not less than one foot passing by. Adjacent flange is starting to fail as well.
