During review of the bridge log, engine room log, and Annex VI logbook recording the fuel oil changeover operations, the inspectors noticed inconsistencies regarding the position of the ship when the fuel oil changeover operation was completed during an outbound transit from the US 04OCT15. The Coast Guard questions the authenticity of the above listed logs as presented and the crew at the time's compliance with MARPOL Annex VI regarding the prevention of pollution by emissions.

Resolution: The crew has since changed and the attending inspectors found no apparent ECA violations since October 2015. It appears the incident was limited to one occasion.

Compliance of the ship with the requirements of the International Gas Carrier Code should be shown in the International Certificate of Fitness for the Carriage of Liquefied Gases in Bulk. Port State Control Officers noted that the vessel's Certificate of Fitness does not list "Mixed C4" as an authorized cargo. Both cargo tanks were loaded with "Mixed C4" at the time of Port State Control exam.
