Whenever a defect is discovered, which affects the safety of the ship or other surveyed equipment, the master or owner of the ship shall report at the earliest opportunity to the administration or recognized organization. A wooden wedge was found being used to stop a leak in the steam dump condenser salt water outlet, a flange was found severely leaking on the No. 1 SGW heat exchanger’s salt water outlet, and a Belzona patch was found on the sprinkler piping in the on-deck cargo gear/sampling locker.
