SPRING BREEZE

The name of the vessel must be clearly marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. MI observed vessel name was damaged on the starboard bow of the vessel. Additionally, neither the name nor hailing port were marked on the stern. 46 CFR 67.123(a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. MI observed fractured welds on supports for the aft folding ladder located on the transom. The first location was on the port diagonal support frame located on the exterior of the vessel where the support meets the hull. The second location was on the inboard support frame located on the interior of the vessel where the support meets the deck. 46 CFR 176.700

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Battery for the Ring Life Buoy light is expired. 46 CFR 180.70(d)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Placard that was posted was faded beyond the point of legibility. 46 CFR 185.516(a)

The battery lead must have a fuse in series as close as practicable to the battery. An overcurrent protection fuse was not installed between the battery and distribution panel 46 CFR 183.350(g)

Each vessel must be marked as required by part 67, subpart I, of this chapter. The vessel name on the starboard hull is not legible. 46 CFR 185.602(a)

Each life buoy must be marked in clear legible block capital letters with the vessel's name. The (01) life buoy was found without the vessel's name marked on it. 46 CFR 185.604(b)

Each child lifejacket must be stowed in a location that is appropriately marked and separated from adult lifejackets. The child lifejacket location was not appropriately marked. 46 CFR 180.78(a)(5)

An adult lifejacket must be provided for each person carried on board the vessel. Identified six lifejackets in poor condition. The operator destroyed the lifejackets in the presence of the MI. 46 CFR 180.71

A Certificate of Inspection is valid for 5 years for all other vessels. Upon administrative review, MI discovered vessel overdue for renewal inspection. 46 CFR 176.107 (b)

A vessel on a lakes, bays, and sounds, or rivers route must carry three hand red flare distress signals and three hand orange smoke distress signals, or six hand red flare distress signals, approved in accordance with 46CFR160. One of the six hand red flare distress signals on board was wet and unserviceable. Replace. - 46CFR180.68(b)

In systems and applications where flexible hoses are permitted to be clamped, double hose clamping is required where practicable. Hose leading from fuel tank to fuel filter was missing a hose clamp. Replace. - 46CFR182.410(d)(1)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First-aid kit was missing ammonia inhalants, burn ointment, and eye wash. Replace. - 46CFR184.710

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b) and other provisions of this section. Portable fire extinguisher used for operating station was incorrect rating, discharged, corroded, and unservicable. Additionally other fire extinguishers were incorrect rating for their intended purposes. Permanently mount 40-B:C extinguisher near outboard engines, 10-B:C extinguisher near operating station, and 2-A extinguisher in passenger accommodation space. - 46CFR181.500(a)

A vessel is inspected for compliance with the standards required by this subchapter. Crack discovered starboard side of bow, below the waterline. Grind back to good material and repair. - 46CFR176.800

The design, construction, and stowage of portable tanks and related fuel lines and accessories must meet the requirements of ABYC H–25. Vessel representative was unable to prove portable fuel tank meets ABYC H-25. Provide documentation or replace with approved tank. - 46CFR182.458(b)

A vessel is inspected for compliance with the standards required by this subchapter. Crack weld discovered in base of hinge on rescue platform. Repair. - 46CFR176.800

The owner or operator shall instruct each crewmember, upon first being employed and prior to getting underway for the first time on a particular vessel and at least every 3 months, as to the duties that the crewmember is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. Crew training shall be logged for review by the Coast Guard upon request. The training entry shall include the following information: Date of the training and general description of the training topics. No record of training. Train each crewmember and provide updated training record to USCG. - 46CFR185.420

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked ‘‘First-Aid Kit’’. A first-aid kit must be easily visible and readily available to the crew. First-aid kit has expired aspirin, eye dressings, antiseptic wipes, and burn gel. Replace expired contents and ensure kit contents meet 46CFR160.041. - 46CFR184.710

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: ‘‘DISTRESS SIGNALS’’. No container on board for 3 of the 6 flares. Provide. - 46CFR185.614

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Marine employer has no alcohol testing strips available for testing. Provide. - 46CFR16.240

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer’s policy regarding drug and alcohol use in the workplace. Owner had no record of signed substance abuse policy for each employee as required by program. Provide record of signed policies for each employee to USCG. - 46CFR16.401(a)

An EAP training program must be conducted for the employer’s crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer’s supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. Owner had no record of training for crewmembers or supervisor. Provide record of training to USCG. - 46CFR16.401(b)

Ring life buoy must be fitted with a lifeline that is buoyant, of at least 60 feet in length, non-kinking, have a diameter of at least 5/16 inch, have a breaking strength of at least 1,124 pounds, and be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Lifeline installed does not meet the specifications listed above. Replace. - 46CFR180.70(c)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q. Waterlight battery is incorrect type and markings are too faded to determine make/model of waterlight to find compatible battery. Replace waterlight and battery.
