The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. - 46 CFR 176.700(b) MI observed severe damage on vessel's bow as a result of an allision with a bridge. A repair proposal must be submitted and approved prior to alterations/work. Final repairs shall be inspected by a qualified MI and prior to the carriage of passengers.

Prior to the carriage of passengers, final repairs shall be inspected by a qualified MI and include an operational test of the engine and steering system while underway.

When necessary for safe operation, the cognizant OCMI may place specific stability restrictions in a stability letter or on the Certificate of Inspection of a vessel not more than 65 feet (19.8 meters) in length, which, under § 178.310 of this part, complies with the requirements of § 178.320 of this part. Inspector identified forward spaces with deck inspection hatches that were not sealed watertight. 4" of standing water was found in the forward spaces on both port/stbd side. The operator removed the water during the inspection using a portable pump, however, the inspection hatches do not seal in accordance with the restrictions on the stability letter. Seal the forward space inspection hatches watertight prior to operating beyond a L,B,S route. 46 CFR 178.210

Resolution: The operator pumped the port/stbd fwd spaces dry and removed the previous 10" wide flush inspection opening screw down hatch to the space. The 10" wide inspection opening screw down hatch was replaced in kind and the exterior mount was screwed and sealed.

Each lifeline on a ring life buoy must Be buoyant; Be of at least 18.3 meters (60 feet) in length; Be non-kinking; Have a diameter of at least 7.9 millimeters ( 5/16 inch); Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Inspector found 50' of lifeline attached to the ring buoy.

Inspector identified corroded conductors and corroded fuses in the starboard aft compartment. Two fuses for the battery charger found corroded and one primary branch fuse found corroded. 46 CFR 183.200
