Means shall be provided where by normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Vessel's #1 propulsion engine became overloaded and tripped offline, causing loss of propulsion. The other two operating propulsion engines failed to carry the required load to power the propulsion motors.

Resolution: Vessel submitted Class Report attesting to the complete replacement of the vessel's #1 propulsion engine governor with a new one with updates PMS system software.

Oil fuel pipes, which if damaged, would allow oil to escape from a storage settling or daily service tank having a capacity of 500L and above situated above the double bottom shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. Vessel's emergency generator fuel tank stop valve failed to operate from a remote location.

When electrical equipment is installed in gas-dangerous spaces or zone as provided in 10.1.4, it should be to the satisfaction of the Administration and approved by the relevant authorities recongized by the administration for operation in the flammable atmosphere concerned. Multiple lighting fixtures on aft deck by master gas valve within hazardous location without ceritifed safe fittings.

Fire protection system and appliances shall be maintained and ready for use. Fire main piping above overhead at upper deck enterance was found leaking during fire main test. Upon further investigation, three patches to that section of piping was found, one which had failed.

MGV that can be closed from within the machinery space should be provided in the cargo area. The valve should be arranged to close automatically if leakage of gas is detected, loss of ventilation for duct/casing or loss of pressure in double wall gas piping occurs. This valve was located outside of the cargo area. SOLAS(14)VIII/13.1 IGC 16.3.7

Resolved 12 January 2018
Resolution: Received a valid International Certificate of Fitness with an equivalency issued IAW Section 1.4 of the 1993 IGC Code attesting to an equivalent level of safety regarding the Master Gas valves and the requirements of Section 16.3.7 and 16.3.8 of the code.
