The Master, Owner, Operator in charge of a vessel equipped with Ballast Water tanks must remove fouling organisms from the vessel's hull on a regular basis. PSCO observed excessive fouling around the vessels hull.

Maintenance, testing and inspection of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. PSCO was unable to verify expirations dates on all hydrostatic releases. Ship's crew unable to provide documentation showing age or installation of hydrostatic releases, on Port STBD and FWRD life rafts.

WHEN THE FIRE EXTINGUISHING MEDIUM IS STORED OUTSIDE A PROTECTED SPACE, IT SHALL BE STORED IN A ROOM WHICH IS LOCATED BEHIND THE FORWARD COLLISION BULKHEAD, AND IS USED FOR NO OTHER PURPOSES. SPACES WHICH ARE LOCATED BELOW DECK OR SPACES WHERE ACCESS FROM THE OPEN DECK IS NOT PROVIDED SHALL BE FITTED WITH A MECHANICAL VENTILATION SYSTEM DESIGNED TO TAKE EXHAUST FROM THE BOTTOM OF THE SPACE AND SHALL BE SIZED TO PROVIDE ATLEAST 6 AIRCHANGES PER HOUR. PSCO OBSERVED VENTILATION SYSTEM IN CO2 SPACE WOULD NOT ENERGIZE.
