Every tanker shall be provided with the means to enable the crew to gain safe access to the bow even in severe weather conditions. PSCO's observed that grates on the upper cat-walk near the bow of the vessel were damaged and unable to be properly secured. 74 SOLAS (20) II-2/3-3.2

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use, PSCO's observed that the painter line on the forward life raft was attached to the packing band, thus rendering the life raft not ready for its intended use. PSCO's observed that the quick release life buoy was not attached to the smoke signal, rendering the smoke signal not ready for its intended use. 74 SOLAS (20) III/20.2
