0699- Not less than one half of the total number of lifebouys shall be provided with lifebouy self-igniting lights complying with the requirments of paragraph 2.1.2 of the code. Not less than two of these shall also be provided with lifeboy self-activating smoke signals complying with the requirements of paragraph 2.1.3 of the code and be capable of quick release from the navigation bridge. Self-activating smoke signal was found to be discharged with activating line glued back into place. 74SOLAS(14)CH. III/7.1.3.1

On November 22, 2016, the chief engineer provided an entry in the official oil record book for the use of the oily water separator that exceeds the maximum throughput of the oily water separator system per the approved IOPP certificate. Rectify to the satisfaction of the RO/RSO and Coast Guard.

0220 - Certificates of Competency: The capacity in which the holder of a certificate is authorized to serve shall be identified in the form of an endorsement in the terms identical to those used in the applicable safe manning requirements of the administration. The master's Marshall Island Certificate of Competency was not valid for service aboard ECDIS equipped vessels (STCW I/2.9).

0735 - Personal Equipment - Fire Fighting: Ships shall carry two fire fighting outfits. For tankers, two additional fire fighting outfits shall be required. Three of four SCBA masks were inoperable (SOLAS II-2/10.2.3).

2020 - Fire Drills: Each fire drill shall include checking of firemans outfits. Crew unable to perform fire drill due to failure of operational readiness of equipment (SOLAS III/19.3.4.2.3).

0663 - Immersion Suits: An immersion suit of an appropriate size shall be provided for every person assigned to crew the rescue boat. 13 out of the 17 crewmembers had immersion suits that were too large (SOLAS III/7.3).
