07126 - FIRE SAFETY - OIL ACCUMULATION IN ENGINE ROOM - THE PURPOSE OF THIS REGULATION IS TO PREVENT THE IGNITION OF COMBUSTIBLE MATERIALS OR FLAMMABLE LIQUIDS. FOR THIS PURPOSE, THE FOLLOWING FUNCTIONAL REQUIREMENTS SHALL BE MET: MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS. PSCO OBSERVED THE ACCUMULATION OF FUEL OIL IN VARIOUS LOCATIONS THROUGHOUT THE ENGINE ROOM, TO INCLUDE AROUND AND IN BETWEEN ALL CYLINDERS OF THE MAIN ENGINE, UNDERNEATH GENERATORS 1,2, AND 3, IN A BUCKET UNDER THE BOILER F.O. FEED PUMP UNIT, AND IN A CUP UNDER THE SAMPLING LINE OF THE NO.2 HFO SETTLING TANK. AN ACTIVE FUEL OIL LEAK WAS OBSERVED FROM A FUEL OIL LINE ON THE NUMBER 2 GENERATOR. PSCO OBSERVED LUBE OIL ACCUMULATION UNDER GENERATORS 1,2, AND 3, AND IN A BUCKET UNDER THE M/E NO. 2 L.O. PURIFIER. ADDITIONALLY, PSCO OBSERVED ACCUMULATION OF HYDRAULIC OIL AROUND THE NO.1 STEERING PUMP AND STEERING GEAR, AS WELL AS IN A CUP UNDER A HYDRAULIC LINE NEXT TO THE EMERGENCY GENERATOR HYDRAULIC STARTER. 74SOLAS(20) II-2/4.1.1 - 17AC

0936-Steering Gear-Hydraulic power-operated steering gear shall be provided with the following: a low level alarm for each hydraulic fluid reservoir to give the earliest practical indication of hydraulic fluid leakage. Audible and visual alarms shall be given on navigation bridge and in the machinery space. PSCO observed steering gear hydraulic expansion tank below 25 percent and unable to achieve adequate pressure to operate, system had no active alarms. CE directed tank to be manually filled from day tank and conducted sat operation test. expanded exam revealed low level alarm to expansion tank was INOP and later fixed by the crew. SOLAS 74 a 2014 Cons II-1/29.12 10c
