Vessel failed to implement their SMS by not reporting to the vessel's DVM. SMS states, 10.1.5, unexpected maintenance items are to be reported to the DVM as soon as possible, with consideration given to urgency.

PSCO observed BWTS valve BA088 not properly functioning. PSCO observed fresh water generator outtake valve not installed due to maintenance. this is the only means to create fresh water while at sea.
