A vessel on a great lakes route must carry 6 handheld red flares and 6 handheld orange smoke signals. The vessel are not outfitted with the proper number of distress signals.

Firefighting drills and training shall be logged or documented for review by coast guard. Firefighting drills are not being logged.

The owner shall instruct each crewmember upon first being employed, prior to getting underway for the first time on a particular vessel and at least once every 3 months on the duties and responsibilities listed in 46 CFR 185.420.

Provide FCC radio station authorization for both vessels.

A vessel on a oceans, coastwise, limited coastwise, or great lakes route must carry: (1) Six hand red flares and (2) Six hand orange smoke distress signals. Each vessel has only three hand red flares and three orange smoke distress signals.

Each U.S. flag vessel subject to this subpart must have an inspection of the required radiotelephone installation at least once every 48 months. This inspection must be made while the vessel is in active service or within not more than one month before the date on which it is placed in service. Vessel unable to provide evidence of Great Lakes Radio Agreement inspection. Provide evidence of inspection to marine inspector.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Portable extinguishers are not being inspected and recorded monthly. Inspect and record portable extinguishers monthly as per NFPA 10.

A portable hand bilge pump must be: (1) Capable of pumping water, but not necessarily simultaneously, from all watertight compartments; and (2) Provided with suitable suction hose capable of reaching the bilge of each watertight compartment and discharging overboard. Current hand bilge pump will not enter portholes due to seating arrangement.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. (d) Aluminum hull vessels: (1) Lloyd's Yachts and Small Craft; or (i) For a vessel of more than 30.5 meters (100 feet) in length: ABS Aluminum Vessel Rules (incorporated by reference, see 46 CFR 175.600); or (ii) For a vessel of not more than 30.5 meters (100 feet) in length: ABS Steel Vessel Rules (<61 Meters), with the appropriate conversions from the ABS Aluminum Vessel Rules; or Several seat stanchions are fractured from water entry and then freezing. Reweld all stanchions and drill drain holes to prevent future damage.

Portable fire extinguishers must be inspected & maintained IAW NFPA 10. the portable fire extinguishers are not being inspected monthly as required. Inspect all portable extinguishers montly.

Provide pre-employment drug tests results for Ewa Goldman and Urszula Lewandowski. These persons are restricted from operting in safety sensitive positions on the vessel until a negative pre-employment drug test has been received and persons are enrolled in the random drug testing program.

Each lifeline on a ring buoy must be 60ft in length. Provide ring buoy lifeline w/ 60ft lengths. (5/16th diameter, dark color or UV resistant, buoyant, non-kinking).

Prior to Ewa Ciepielewska, Johnathan Kapelko, Rafal Les, [name withheld] serving as members of the crew. Drills must be conducted with these members in the presence of a Coast Guard Marine Inspector.

All vessels must be fitted with a magnetic compass. Provide an illuminated magnetic compass mounted at the primary operating station.
