Equipment and arrangements for pilot transfers shall comply with this regulation and due regard shall be paid to the standard adopted by the organization (A889.21). PSCO observed photographic evidence that required stanchions, handrails or ropes were not installed. Pilot ladder was not rigged adjacent to lower platform of accommodation ladder but was rigged so as to be obstructed by the platform. Magnet used to secure accommodation ladder was not affixed to ship. All steps on pilot ladder not horizontal due to being fouled by line. Provide satisfactory evidence of compliant pilot ladder arrangements on both sides of vessel prior to departure.

Oil filtering equipment shall be of a design approved by the administration and shall be such as will ensure that any oily mixture discharged into te seas after passign through the system has an oil content not exceeding 15 PPM. The oil content meter's data logger is inconsistent with the oil record book. The oil record book shows on June 8, 2020 that the OWS ran for approximately 5 1/2 hours. The OCM showed the OWS ran for approximately 20 minutes. June 5, 2020 and June 11, 2020 were inconsistent as well.
