Gauges to indicate jacket water discharge temperature must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. STBD engine jacket water temperature gauge at the operating station is not working properly.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Electronic charts have not been updated in many years.

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. The name on the PORT and STBD bow are missing the number 2. The stern of vessel is missing the hailing port.

Each portable fire extinguisher on a vessel must be of a type approved by the Commandant. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI, but must be not less than the minimum number required by table 1 to § 118.500(b) and other provisions of this section. Vessel does not have the minimum number of portable fire extinguishers required on-board the vessel. Multiple portable fire extinguishers were last annually inspected in 2024. Portable fire extinguishers are not stowed IAW approved fire control plan. 46CFR118.500

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. The fire control plan does NOT specify the fire detection system zones and/or spaces protected. 46CFR116.530

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection and testing of smoke and fire detecting systems (including sensors and alarms) and fire confining appliances (such as fire screen doors and fire dampers). Unable to proceed with witnessing testing of smoke and fire detecting system, due to inadequate designation of spaces programmed on the panel, all vessel spaces are designated as "Zone 1" on the control panel. To proceed with system testing, the control panel must be properly programmed to specify each individual dectector name, ID, space, etc. 46CFR115.810

Each elevator on a vessel must meet the requirements of American National Standards Institute (ANSI) A17.1. “Safety Code for Elevators, and Escalators,” or other standard specified by the Commandant. Provide annual elevator inspection certificate from a certified technician. 46CFR120.540

Elevator does NOT return to the embarkation deck (main deck) upon loss of electrical power, it only returns to one deck below the current elevator car location upon a loss of electrical power. 46CFR120.540

Unless otherwise stated in this section, life jackets must be stored in convenient places distributed throughout accommodation spaces. An OCMI determination was made that current stowage of life jackets are not acceptable. Life jackets are not readily available at the embarkation deck, as the current stowage locations are on decks 1, 3, 4, and 5. 46CFR117.78

Each elevator on a vessel must meet the requirements of American National Standards Institute (ANSI) A17.1. “Safety Code for Elevators, and Escalators,” or other standard specified by the Commandant. During the elevator system loss of power testing, the UPS for the elevator system is non-functioning. Replace UPS for the elevator system IAW manufactures instructions and specifications. Then prove proper operation of elevator system, including loss of power to the attending CG Marine Inspector. 46CFR120.540

Except when in the judgment of the cognizant OCMI the operation of a vessel does not present one of the hazards listed, the emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 122.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions don't address the procedures for passengers donning life jackets stowed on the 3rd, 4th, and 5th decks, then abandon ship from the embarkation deck IAW the approved fire control plan. Update emergency instructions. 46CFR122.510

The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions don't address the procedures for the elevator system emergency egress upon loss of power. Update emergency procedures to include procedure for the elevator system loss of power, and procedures for the elevator to return to the designated deck (embarkation deck) upon loss of power, and conduct and document full crew training concurrent with the fire drill periodicity. 46CFR122.510

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel must have a written procedure for inspecting and testing the elevator system IAW manufactures instructions and specifications within the manufacturers recommended intervals. 46CFR115.840

The development of a nontank vessel response plan prepares the vessel's crew and ship management to respond to an oil spill. Vessel does not have an authorized Nontank Vessel Response Plan. Vessel required to maintain a Nontank Vessel Response Plan in accordance with 33 CFR 155 Subpart J because it is greater than 400 GT ITC. 33CFR155.5015

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: the radio operator must hold a marine radio operator permit or higher class license. Provide evidence of Marine Radio Operator Permit to the USCG. 47CFR80.159(e)

A fire drill must be conducted at least once each month. Vessel is not currently operating and does not have required crew manning, therefore vessel has not conducted a fire drill. Vessel must conduct a fire drill witnessed by the USCG. 46CFR122.524

Each member of the crew shall participate in at least one abandon ship & man overboard recovery drill each month. Vessel is not currently operating and does not have required crew manning, therefore vessel has not conducted abandon ship & man overboard recovery drills. Vessel must conduct abandon ship & man overboard recovery drills witnessed by the USCG. 46CFR122.520

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Vessel VHF radios are programmed with prior MMSI numbers. Reprogram VHF radios to updated MMSI numbers as documented on vessels current FCC station license, then perform digital selective calling test call (DSC) IAW MSIB 20-20 in the presence of the attending CG Marine Inspector. 46CFR121.502

The vessel mast and stern navigation lights are inoperable. Make permanent repairs to the vessel mast and stern navigation lights to the satisfaction of the attending CG Marine Inspector. 33CFR83.20

All escape hatches and other emergency exits used as means of escape must be marked in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant OCMI. Not all emergency exits leading from the crew spaces on the lower deck to the main deck are properly marked with the correct verbiage and marking size. Properly mark all escape hatches and other emergency exits to the satisfaction of the attending CG Marine Inspector. 46CFR122.606

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Life jacket placards are not for the type carried on board the vessel. Post life jacket donning instruction placards for the type of life jacket carried on board the vessel. 46CFR122.516

An adult life jacket must be provided for each person carried on board a vessel. The vessel is required to carry 520 adult sized life jackets. 71 adult sized life jackets are excessivly deteriorated and are in unsatisfactory condition. Vessel is currently carrying 449 satisfactory condition adult sized life jackets. Vessel may not carry more than 449 total persons until additional satisfactory condition adult sized life jackets are carried on board the vessel. 46CFR117.71

Each fire hose must:(1) Be lined commercial firehose that conforms to UL 19 “Standard for Safety for Lined Fire Hose and Hose Assemblies” (2) 50' feet in length, 1.5" in diameter; (3) Have fittings of brass or other suitable corrosion-resistant material that comply with NFPA 1963. 46CFR118.320

Vessels must have an interconnected fire detection system in compliance with 46 CFR 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Except for continuously manned operating stations as allowed by paragraph (f) of this section, each accommodation space, control space, and service space must be fitted with the following systems:(1) A smoke actuated fire detection system of a type approved by the Commandant that is installed in accordance with 46 CFR part 76; and(2) A manual alarm system that meets the requirements in 46 CFR part 76. The fire detection system is inoperable. Troubleshoot and repair fire detection system to manufacturers specifications, then test and prove proper operation of all system components to the satisfaction of the attending CG Marine Inspector. 46CFR118.400

A vessel must be provided with a bilge piping system that meets § 56.50-50 in subchapter F of this chapter. The bilge piping system cannot take suction from the aft crew space. Repair bilge piping system and prove proper operation to the satisfaction the attending CG Marine Inspector. 46CFR119.510

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. MI observed outdated information.

A vessel of 12 meters or more in length shall be provided with a whistle, a vessel of 20 meters or more in length shall be provided with a bell in addition to a whistle. MI observed ship's whistle inop.

A vessel of more than 19.8 meters (65 feet) in length must carry a minimum of three life buoys of not less than 610 millimeters (24 inches) in diameter. MI observed vessel name worn off.

Hazardous situations must be corrected and all protective devices must be in satisfactory condition. All accesses (1 closet and 2 lockers) shall be locked/secure to prevent passenger access to the spud winches/operation.

A vessel must comply with radio requirements set forth by FCC in 47 CFR 80. Vessel shall provide and post required FCC docs.

Abandon ship man overboard, and fire drills shall be performed at least once a month. Company shall ensure policy changes and drills are performed and documented.

Provide proof of installation of a Class A AIS and removal of the existing Class B

Provide vessel with magnetic compass.

Provide vessel with emergency instructions.
