After any survey of the ship under regulation 8 has been completed, no change shall be made in equipment and other items covered by the survey, without the sanction of the administration. The company and ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the code shall be treated as mandatory. The ship failed to report non-conformities addressing spare parts required to repair a fuel leak on number 2 diesel generator, based on their own written SMS procedures. 74SOLAS(20)IX/3.1 & ISM PART A/9.2

02134- To discharge ballast water into the water of the United States, the master, owner, operator, agent, or person in charge of a vessel subject to 33 CFR 151.2025 must either ensure that the ballast water meets the ballast water discharge standard in 151.2030(a), use an AMS as described in 151.2025(a)(3), or ballast with water from a U.S. public water system, according to the schedule in paragraph (b) of this section. M/V V UNO keel laid date 03 MAR 2014 and ballast water capacity 11,300 m3, is past its compliance date per the implementation schedule on 33 CFR 151.2035(b). The vessel's ballast water treatment system is inoperative. 0735 Personal equipment-fire fighting - A firefighter's outfit shall consist of personal equipment. Personal equipment shall consist of protective clothing of material to protect the skin from the heat radiating from the fire and from burns and scalding by steam.
