A small passenger vessel of not more than 65 feet must have one ring life buoy. The life ring buoy must be fitted with a floating water light approved in accordance with 46 CFR 160.010 and properly secured around the body of the life ring buoy in accordance with 46 CFR 180.70(c). The water light was stowed in the upright position and the battery was corroded rendering the light inoperable. Provide and properly secure a new floating water light on the Life Ring Buoy to the satisfaction of a Coast Guard Marine Inspector no later than September 20, 2024

A Certificate of Inspection may be suspended and withdrawn or revoked by the cognizant OCMI at any time for noncompliance with the requirements of this subchapter.This vessel has four (04) outstanding deficiencies that were issued to the owner on May 17, 2023. No action has been taken to correct these deficiencies. The COI has been deactivated and the vessel is not allowed to carry passengers until alkl deficiencies have been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels pre-engineered (Fireboy) fire extinguishing system protecting the machinery space does not shut down the generator and does not meet the minimum standard of automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space in accordance with 46 CFR 181.420(a)(3). The vessel is restricted from carrying passengers until corrected to the satisfaction of an attending Coast Guard Marine Inspector.

An adult life jacket must be provided for each person carried on board a vessel. Provide Type 1 life jackets for total persons on board as allowed by your COI. Vessel is restricted to 48 total persons on board. 46CFR180.71(a)

Provide proof of pre-employment drug testing and enrollment in the Company's random drug testing pool for all employees in a sensitive safety position in accordance with 46 CFR 16.210. Members (Clint Adkins and Chris Jacobs) are not authorized to fill any safety sensitive position on the vessel until the necessary proof of pre-employment and enrollment in the random program are received and reviewed by this office

Vessels first aid kit is expired and does not meet the minimum requirements per 46CFR184.710 and 46CFR 160.041. Vessel has 30 days to correct to the satisfaction of the CG Marine Inspector. The vessel will be restricted from carrying passengers after June 17, 2023 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels Fire bucket lanyard and did not meet the minimum requirements as described in 46CFR181.610. Vessel must have at least three 9.5 liter (2 1 /2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. 46 CFR 181.610. Provide new attached lanyards. The vessel will be restricted from carrying passengers after June 17, 2023 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The markings required be made by the use of any means and materials which result in durable markings, must be made in clearly legible letters of the Latin alphabet or Arabic or Roman numerals not less than four inches in height. 46CFR67.123 Ensure vessel name is marked. The vessel will be restricted from carrying passengers after June 17, 2023 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels machinery space ventilation does not have a closure device and does not meet the minimum standard of provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent as described in 46CFR182.460. The vessel will be restricted from carrying passengers after June 17, 2023 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

EAP training program: An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. 46CFR16.401. No evidence of training program or 60 minutes of supervisory training. The vessel will be restricted from carrying passengers after June 17, 2023 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

A flammable vapor detection system must be operational for 30 seconds prior to engine startup and continue sensing the entire time the engine is running. 46CFR182.480( c ). Vessel was observed with engine running underway and the vapor detection system off. Was corrected on the spot.

COD is expired.

Collision bulkhead penetrations are not sealed properly.

Forward lifebuoy throw line does not meet the required length.

The aft main deck lifebuoy outer covering is damaged.

The high water level indicator midships is wired in with a extension cord.

Vessels bilge high level alarm in mid compartment were not operating properly and does not meet the standard of visual and audible alarm must be provided at the operation station per 46CFR182.530(a). The vessel will be restricted from carrying passengers after September 3, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels food heater and refrigerator were not securely fastened to the bulkhead and at risk of tipping over and does not meet the minimum requirements as described in 46CFR176.830. The vessel will be restricted from carrying passengers after September 3, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Vessels starboard side ventilation duct is combustible and does not meet the minimum requirements of 46CFR182.460. Replace duct with a non combustible material. The vessel will be restricted from carrying passengers after September 3, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Properly mark the name of the vessel in at least four inch block letters on both sides of the bow in accordance with 46 CFR 67.123

Ensure all electrical outlets located on the weather decks in exposed locations have suitable watertight covers for protection against shock and damage in accordance with 46 CFR 183.210(b) This deficiency was corrected prior to completion of the inspection.

Provide one additional portable fire extinguisher to protect the second deck passenger accommodation in accordance with the table in 46 CFR 181.500(b). Fire extinguisher must be classified as a 2-A. This deficiency was corrected prior to completion of the inspection.

Install a new high level alarm float and demonstrate proper operation of the high level alarm, both visual and audible at the operating station in accordance with 46 CFR 182.530(a)(2) to the satisfaction of an attending Coast Guard Marine Inspector.

Prior to credit for dry dock, install valves on the open through hull fittings in the sea chest located above the water line to prevent potential progressive down flooding to the satisfaction of an attending Coast Guard Marine Inspector.

Prior to credit for dry dock, remove three (03) plastic dead end caps in way of the thru hull fittings and replace with metallic, threaded caps to the satisfaction of an attending Coast Guard Marine Inspector.

Prior to credit for dry dock, properly secure all loose and hanging wires and cables as identified throughout the vessel with metallic supports spaced not more than 24 inches apart in accordance with 46 CFR 183.340(b)(4) and to the satisfaction of an attending Coast Guard Marine Inspector.

Develop procedures to ensure an accurate, written count of all passengers is recorded on the vessel. Prior to departure on the voyage, the passenger count, including the number of crew members, must be communicated ashore and available to the Coast Guard upon request.

Prior to getting underway with passengers on board, the Master or another member of the crew, must make a public safety announcement and address all safety items as outlined in 46 CFR 185.506(a).

Ensure the first aid kit has all the required contents in accordance with 46 CFR 160.041 and replace all expired medications. Deficiency has not been cleared. 835 has been extended until August 5, 2016.

Post near the operating station Emergency Instructions containing the actions to be taken in the event heavy weather, fire and man overboard. The Master and crew must be familiar with the content of the posted Emergency Instructions.

Ensure all emergency drills (Fire, Abandon Ship and Man Overboard) are conducted and logged with all crew at least once every three months. Ensure emergency drills are conducted prior to getting underway with passengers after the vessel has been laid up for the off season.

Mark the number on each life jacket storage locker with the total number of life jackets stowed within.

Provide a floating water light in way of the life ring buoy that is approved under 46 CFR 161.010. The water light must have a lanyard of between 3 to 6 feet with a corrosion resistant clip and attached around the body of the buoy. In addition, the life ring buoy must be fitted with a life line of at least 60 feet.

Remove all electric submersible bilge pumps that are not UL approved and provide a bilge bump in each watertight compartment that meets the UL 1113 standard. Deficiency has been extended an additional 30 days because of the lack of availability of the UL Approved Rule pumps.

Provide a positive action valve at the hull penetration in way of the discharge hose for each electric submersible bilge pump in accordance with 46 CFR 182.520(e)(7). Deficiency has been extended 30 days because the new pumps were just received by the owner.
