Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers (incorporated by reference, see § 136.112 of this subchapter), with the frequency specified by NFPA 10 and as amended here: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. The monthly inspections of the portable and semi-portable extinguishers were not being conducted and records in towing vessel record (TVR), conduct monthly inspection and record inspection in TVR. 46 CFR 142.240(a)(1)

Each towing vessel must be fitted with a general alarm that activated at each operating station and can notify persons onboard in the event of an emergency. Fire detection panel were disabled and showing faults. Faults were cleared and panel enable by vessel representative. 46 CFR 143.235(b)(1)

Resolution: Vessel's representative cleared faults and reset system to normal operating status.
