An audible and visual alarm system to give a warning on both sides of the airlock should be pro vided to indicate more than one door is moved from the closed position. Port State Control Officers discovered that neither visual or audible alarms activated inside or outside the space when both airlock doors were moved from the closed position. After this finding the crew replaced two fuses. The fuses burned out after testing airlock for second time. The fuses had been replaced with 2 amp instead of 3 amp. Once correct fuses were installed the airlock alarms operated satisfactory.

-1160- HOLDS AND TANKS - Cargo tank liquid level gauge closed devices which penetrates the cargo tank but which form part of a closed system and keep the cargo from being released. Incomplete fusion; 8 inch separation of weld from based of liquid level gauge to tank dome was found on No.3 cargo tank, resulting in leakage of ethylene cargo.
