The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or recovery of persons who have fallen overboard. The master shall conduct sufficient fire drills to ensure that each crew member is familiar with his or her duties in case of a fire. Vessel crew members did not demonstrate adequate proficiency in practically executing the vessel's revised Emergency Evacuation Plan and Abandon Ship Plan during the fire and abandon ship drill (scenario: fire in 5th deck guest laundry that spread to the stairtower) held during the inspection. Additional crew training to be conducted, with follow-on additional fire and abandon ship drills to be carried out to the satisfaction of the attending CG Marine Inspectors. 46 CFR 122.520 46 CFR 122.524

Doors in bulkheads required to be Class A-60...shall be of hollow steel or equivalent metal construction solidly filled with approved structural insulation capable of meeting the requirements for a Class A-15 bulkhead. Gaskets on fire doors for main laundry and the engine room were found to be missing or deteriorated, leading to these A-class doors not being arranged to prevent passage of flame and smoke as required. 46 CFR 72.05-25 (b)

Waste paper baskets shall be constructed of approved incombustible materials with solid sides and bottoms. Waste receptacles in the galley, main laundry, and other various locations were made of combustible materials. 46 CFR 116.400(b) NVIC 9-97 CH-1, Section 2.9.2 (pg37)

Suitable means, such as a manual damper, automatic damper, or vent cover, must be provided in an accessible location outside the space served by the ventilation duct for shutting off the passage of air through the ventilation duct in the event of fire. Automatic fire damper for the range hood was found inoperable. Vessel engineered temporary fix (slide in plate on the exterior of vessel). Vessel to affect permanent fix at next drydock. 46 CFR 116.610

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions. Vsl reported loss of # 2 generator due to faulty speed sensor. Complete satisfactory replacement of speed sensor and repairs, and conduct steering test with affected generator as the power source, with CG marine inspector present. 46CFR120.200

Clear instructions must be provided on the vessel that detail the actions each person on board should follow in the event of an emergency. Each muster list must at least specify: (6) the duties assigned to different members of the crew. Duties to be specified include (iii) preparing and launching the survival craft; (7) duties assigned to members of the crew in relation to passengers and other persons on board in case of an emergency. Assigned duties to be specified include: (iii) assembling passengers and other persons on board at muster stations, (iv) controlling the movement of passengers and other persons onboard. The master shall conduct sufficient drills and give sufficient instruction to make sure that all crewmembers are familiar with their duties during emergencies that necessitate abandon ship or recovery of persons who have fallen overboard. The station bill/muster list in use at time of the inspection does not assign several key duties to specific persons, and crew response during observed abandon ship and man overboard drills did not fully correspond with the station bill/muster list or passenger safety bill. Examples include (not all inclusive): station bill assigns the mate to “supervise launching of liferafts”, but no persons are assigned to launching of each individual IBA or moving each 50 person IBA from the stowage location to the embarkation station. Only one crewmember posted at embarkation station to direct passengers into IBAs; not the person listed on the station bill. During the drill, passengers directed from muster station to embarkation station and boarding of IBAs by deck, but passenger safety bill assigns specific IBA # based on stateroom; no clear control on specific passengers and/or crew to embark each IBA. Station bill does not have an assigned helmsman during man overboard; the bridge wing conning stations do not have rudder controls; thereby necessitating a helmsman. Update station bill as appropriate and carry out additional crew training. Subsequently, conduct additional abandon ship drill in presence of attending CG Marine Inspectors.

Passageways and stairway enclosures shall contain only fire resistant furnishings. All free-standing furniture such as chairs shall be constructed of frames of approved incombustible materials. In addition, all upholstery and padding of chairs, sofas, etc. in these areas shall be of approved fire resistant materials. Upholstered chairs with wooden legs found in the category 2 'vestibule'; no markings found on furniture or documentation presented indicating compliance with these standards. Provide objective evidence that this furniture meets the cited requirements (satisfying the requirements in 46 CFR 116.423 (b) is also acceptable) or remove such furniture from the stairtower.

The following emergency lighting […] must be arranged […]: (d) Illuminated signs with the word “EXIT” in red letters throughout a passenger vessel so the direction of escape to the open deck is obvious from any portion of the vessel usually accessible to the passengers or crew. Vessel's approved Fire Control Plan has an "EXIT" sign in the 2nd deck forward corridor (just forward of double doors leading into the vestibule), but "EXIT" sign is missing. In the second deck aft corridor (just aft of the vestibule) the "EXIT" sign is inoperative under DC power. Install missing "EXIT" sign, repair the second. Update 4/15/25: "EXIT" sign in second deck aft corridor has been replaced and is now functioning properly. Due date for installation of "EXIT" sign in second deck forward corridor has been extended to 12/31/25.

The construction and arrangement of a vessel must allow the safe operation of the vessel giving consideration to protection against fire. Sections of vapor barrier covering structural fire protection insulation in category 10 stackhouse space found missing or deteriorated.

Accommodation spaces will be considered as including the following spaces: […] staterooms, including passenger and crew rooms. Service spaces will be considered as including the following spaces: […] storerooms. If the use of a space could fit into two or more categories defined in the tables, the type of space with the highest boundary requirements should be used. Room 119 designated as category 6 accommodation space on approved Fire Control Plan. Space currently being used as supplemental dry stores / storeroom. Existing fire load calculations for "Hull #119 Typical Double Stateroom" are not representative of the current fire load in the space. Demonstrate to the satisfaction of the local OCMI how use of this space will remain consistent with a category 6 accommodation space (vice a category 9 high-risk service space).

The following emergency lighting must be arranged: (c) lighting for passageways sufficient to allow crew to find their way to open decks and muster stations with all fire doors closed. There is no e-light in the category 3 corridor at the exit of the machinery space, dry stores, and galley. Space is virtually pitch black when main power is lost with all fire doors closed. Install an emergency light to sufficiently illuminate the space.

Nonconducting mats or grating must be provided on the deck in front of each switchboard and, if accessible from the rear, on the deck in the rear of the switchboard. Observed 01 non conductive matting missing in auxiliary generator room.

A door in a stairtower, stairway and main vertical zone bulkhead must meet the following additional requirement: A door must be of the self-closing type capable of closing against a 3.5 list of the vessel. Observed fire screen door on 2nd deck passenger embarkation station did not close.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load. Vessel experienced possible fuel injector issue with port engine and shut engine down. Identify mechanical issue with engine, provide technician report attesting to repairs made to USCG Marine Inspector. 46CFR119.200.

All relief valves for use on pressure vessels shall be set to a relieve at a pressure which does not exceed the maximum allowable working pressure of the vessel. Relief valve on air receiver found to be set at 150 psi for a vessel with MAWP of 130 psi. Set relief valve to correct psi and provide proof to CG.

Each PFD light must be permanently and legibly marked with the date of manufacture and expiration date. 02 PFD lights found inoperable. All PFDs found missing battery expiration date. Verify proper operation of all PFD lights onboard and devise a plan to track PFD battery expiration dates.

At each subsequent inspection for certification the owner/operator shall be prepared to conduct tests of the smoke and fire detection system. Crew room 7 was returning a "trouble smoke" error on the fire panel. Rectify error and conduct test on the detector to confirm proper operation, return proof to CG.

A door in a stairtower, stairway, and main vertical zone bulkhead must not have a holdback. Holdback discovered on the door leading the pilothouse. Remove holdback and provide proof to the CG.

A door in a stairtower, stairway, and main vertical zone must be of the self-closing type capable of closing against a 3.5 list of the vessel. Multiple fire screen doors throughout the vessel discovered to be obstructed from closing.

Vessel must submit evidence of developed safe handling procedures for the operation and storage of potentially hazardous items such as rechargeable batteries. Vessel must submit evidence of marine firefighting training programs to improve crewmember training and proficiency, including egress training for each member of the crew.

At each initial and subsequent inspection for certification, the marine inspector may require that a man overboard drill be held under simulated emergency conditions specified by the inspector. Due to weather conditions, vessel unable to get underway to conduct MOB drill. Conduct MOB drill in presence of Marine Inspector. 46CFR115.808(g)

All cable and wire must be installed with metal supports not more than 24 inches apart. Wires hanging without adequate metal supports in emergency steering gear room. 46CFR120.340(b)(4)

Electrical equipment on a vessel must be installed and maintained to protect crew from electrical hazards. Multiple dead end wires found on bridge. provide proof that dead end wires have been removed. 46CFR120.200

Each life jacket must be marked in clearly visible block capital letters with the vessel name. Multiple life jackets not marked properly. Provide proof that life jackets have been properly marked. 46CFR122.604

Each lighting fixture must comply with 46CFR120.200. Lighting fixture cover in emergency steering gear room not installed to protect crew and vessel from electrical hazrads. Ensure lighting fixture cover is properly installed and provide proof to marine inspector. 46CFR120.401

Each generator prime mover must have an overspeed device. Vessel operator unable to perform overspeed test on final emergency power source (Kohler). Provide technician report for overspeed test to Sector Puget Sound. 46CFR111.12-1(b)

Repair fire pump in way of excessive leak located in the engine room.

Vessel needs proper installation of structural fire protection in accordance with fire safety plan. Replace missing structural fire protection over the stern thruster located in the CO2 room.

Repair ground on number two generator.

All escape hatches must be marked on both sides "EMERGENCY EXIT, KEEP CLEAR". Properly mark egress hatches in way of steering gear and CO2 room.

Vessel needs proper installation of structural fire protection in accordance with fire safety plan. Repair modified structural fire protection in way of number one forward bow thruster.

Remote shut-off stations must be marked. Replace missing fuel shut-off placard for the emergency generator room.

Non-metallic flexible hose assemblies must meet the requirements of subchapter F, 46 CFR 56.60-25. Replace the non-metallic flexible hose installed in way of the fire pump.

General alarm system must meet the requirements of Subchapter J, 46 CFR 113.25. Each alarm signal must be marked in accordance with 46 CFR 113.25-20(d). Provide a general alarm placard in way of the bell in the emergency generator room.

Multiple fire doors throughout vessel found to exceed 1/8 inch gap.

All cable and wire must be installed with metal supports spaced not more than 24 inches apart. Power cord for LAUNCH 9 was not properly supported.

Ladders provided to aid escape must be suitable for use in emergency conditions, unless they can be folded, yet brought into immediate service in an emergency. Ladder in crew berthing used as emergency escape was not easily accessible. Leave ladder in the lowered position.

Unable to witness man overboard/fire/abandon ship drills because of full crew not being in attendance due to COVID-19. Vessel required to conduct drills in presence of USCG Marine Inspector prior to the carriage of passengers. 46 CFR 122.520 & 524

Machinery bulkhead are required to be A-0 and machinery decks are required to be A-60 class. Several bulkheads and decks in the engine room were missing the required insulation to qualify them as the proper fire protection.

No more than 55 gallons of combustible liquids may be stowed in any machinery space. Approximately 40 5 gallon buckets of oil were stored in the engine room.

Chemical testing of personnel must be conducted as required by 46 CFR 16 and in accordance with the procedures detailed in 49 CFR 40. The crew was unfamiliar with the Designated Employer Representative, EAP hotline number was not on board, the company's drug and alcohol policy and the 2018 MIS report were not available for inspection. Prior to departing, master obtained EAP hotline number and obtained a copy of the company's drug and alcohol policy.

A vessel of more than 65' in length with overnight accommodations for more than 49 passengers must comply with the requirements of 78.13 in subchapter H of this chapter in regards to the content of a station bill, the duties of the crew, emergency signals, an emergency squad, and instructions...when required more than four crew members, must set forth the special duties and duty station of each crew member for various emergencies. Some crew members were not identified on the station bill.

Each control system for main and auxiliary steering gear and each power actuating system must restart automatically when electrical power is restored after it has failed. CG Marine Inspector observed the following placard in the pilot house at the steering control panel "If hotel power is switched between generators while underway, steering pumps will need to be restarted." Once voltage is restored to steering motor controller, the motor must resume operation on its own, designed with low voltage release.

Vessel does not have an approved nontank vessel response plan. Provide copy of approval letter. 33CFR155.5015
