Vessel experienced high-temp on its Port MDE cylinders #1 and #2 while moored at Stone Dock in Port Fourchon. Vessel is to transit to Bollinger Shipyard in Port Fourchon to affect repairs and notify CG upon completion of repairs.
705 - Vessel is authorized to depart Port Fourchon to only conduct Sea-Trials offshore.

Oil filtering equipment referred to in paragraph 1 of this regulation shall be of a design approved by the Administration and shall be such as will ensure that any oily mixture discharged into the sea after passing through the system has an oil content not exceeding 15 parts per million. In considering the design of such equipment, the Administration shall have regard to the specification recommended by the Organization. Oily water separator's discharge goes into the ship's gray water holding tank. The return line for the OWS goes into the bilge. The ship's IOPP Part 3.3 section list 02 holding tanks for the OWS not being used. The IOPP is incorrect. Vessel shall not use OWS until plans have been submitted to USCG Marine Safety Center or ABS Engineering for approval.

Entries shall be made in the Oil Record Book on each occasion, on a tank to tank basis if appropriate, whenever any of the following cargo/ballast operations take place on any oil tanker to which this section applies— (1) Loading of oil cargo; (2) Internal transfer of oil cargo during voyage; (3) Unloading of oil cargo; (4) Ballasting of cargo tanks and dedicated clean ballast tanks; (5) Cleaning of cargo tanks including crude oil washing; (6) Discharge of ballast except from segregated ballast tanks; (7) Discharge of water from slop tanks; (8) Closing of all applicable valves or similar devices after slop tank discharge operations; (9) Closing of valves necessary for isolation of dedicated clean ballast tanks from cargo and stripping lines after slop tank discharge operations; (10) Disposal of oil residue; and (11) Any failure of, and the reasons for, the oil discharge monitoring and control system. Oil Record Book has not been maintained. Internal and external transfers have not been recorded or properly recorded.
